En France, l'accréditation des laboratoires de biologie médicale selon la norme NF EN ISO 15189 est obligatoire et effective depuis 2013. Concernant les structures d'Anatomie et Cytologie Pathologiques (ACP), l'accréditation n'est pas obligatoire et relève d'une démarche volontaire. Quelques laboratoires d'ACP en France se sont déjà engagés dans cette voie et ont obtenu une accréditation selon la norme NF EN ISO 15189 pour une ou plusieurs technique(s). L'objectif de ce travail est de préciser les bases méthodologiques de la validation des méthodes appliquée à l'ACP et de les mettre en pratique par la réalisation de deux dossiers de validation. Le premier dossier a été réalisé en 2015 à l'Institut Paoli-Calmettes (IPC) et concerne la coloration standard en histologie. Il était réalisé dans le but d'étendre l'accréditation du laboratoire d'ACP, déjà accrédité pour une technique d'immunohistochimie. Le second dossier a été réalisé en 2017 dans le service d'ACPNP du centre hospitalier de la Timone et porte sur la cytologie cervico-utérine en milieu liquide. Il s'agissait du premier dossier de validation réalisé dans ce service et il s'intégrait notamment dans le cadre d'un changement de technique de coloration. La réalisation d'un dossier de validation de méthode comporte de très nombreuses étapes nécessitant une collaboration étroite entre médecins et techniciens. Les étapes les plus importantes sont la planification, l'analyse des risques et facteurs de maîtrise, l'élaboration du plan expérimental et sa mise en œuvre. Pour les 2 méthodes, les tests réalisés ont permis de confirmer la validité opérationnelle des techniques. Le premier dossier a reçu l'évaluation positive du Comité Français d'Accréditation (COFRAC). Les difficultés rencontrées sont essentiellement liées à l'application de la Norme NF EN ISO 15189. Cette norme, destinée aux laboratoires de biologie médicale, est peu adaptée aux spécificités de l'ACP. La démarche est toutefois globalement positive pour les 2 laboratoires. Elle aura notamment permis l'extension d'accréditation du laboratoire d'ACP de l'IPC et un changement de méthode organisé, formalisé et efficace pour le service d'ACP et de Neuropathologie de la Timone. L'accréditation de l'IPC et de 16 autres laboratoires d'ACP en France pour une partie de leurs techniques prouve que cette démarche d'accréditation selon la norme 15189 est possible. Cette norme reste néanmoins mal adaptée à l'ACP et il serait souhaitable qu'un référentiel propre soit élaboré.
