L’évolution des professionnels de santé libéraux de proximité, les bouleversements démographiques, et les nouvelles attentes de la population nécessitent une nouvelle organisation des soins de premier recours. En 2006, des professionnels commencent à décrire des lieux de santé où on retrouverait un travail coordonné entre les différents acteurs, en 2007, la Haute autorité de santé définit ainsi les maisons de santé ; pôles de santé et centres de santé. Pont-Évêque est une commune urbaine de l’Isère située à trois kilomètres de Vienne ; malgré une cartographie non déficitaire, il existe localement une désertification démographique des professionnels de santé sur la commune et ses alentours. En 2009, débute un projet de création d’une maison de santé sur la commune grâce à un des médecins généralistes de la commune et du maire. Malgré le caractère unique de chaque structure, certaines démarches sont indispensables, à travers une expérience personnelle, nous avons voulu définir ces étapes : Etude du territoire, choix des différents acteurs, écriture des projets de santé et professionnel, définition des statuts juridiques, étude du financement, portage du projet immobilier, choix du système d’information. Ces regroupements de professionnels permettent de diversifier les activités : développement de protocoles de soins, mise en place d’éducation thérapeutique personnalisée, formation d’étudiants, travail en réseau avec des partenaires médicosociaux. Leur atout principal est la qualité de fonctionnement et d’activités ; leur faiblesse reste leurs finances : rémunération du fonctionnement de la maison de santé, temps passé pour les actions de prévention, d’éducation thérapeutique. La maison de santé de Pont-Évêque est toujours en projet ; le permis de construire va être déposé en juin 2013, elle ouvrira ses portes en fin d’année 2014.
