Contexte : La coordination des soins est indispensable à la prise en charge des patients. De la qualité de celle-ci dépend une meilleure efficience des soins, ainsi qu'une sécurité optimale pour les patients. L'objectif principal de l'étude était d'analyser les représentations des patients opérés en chirurgie ambulatoire concernant la coordination des soins. Méthode : Une étude qualitative par théorisation ancrée sous forme d'entretiens semi-dirigés a été réalisée de Janvier à Juillet 2014 jusqu'à saturation théorique des données. Les entretiens ont été retranscrits, anonymisés puis codés grâce au logiciel Nvivo®. Résultats : 17 entretiens auprès de patients ayant bénéficiés d'une chirurgie ambulatoire ont été réalisés. Les patients décrivaient une coordination des soins ville-hôpital efficace. Ils exprimaient un sentiment positif concernant la chirurgie ambulatoire, qu'ils considéraient comme rapide, efficace, permettant un gain de temps et économique pour la société. Ils percevaient parfois la sortie comme trop précoce, risquée, avec un retour au domicile compliqué à gérer, après une chirurgie finalement pas si « légère » que cela. Les patients exprimaient un sentiment de « surinformation », régi par un cadre réglementaire ne permettant pas la délivrance d'une information adaptée et répondant à leur besoin. Ils soulignaient un manque d'information concernant ; le déroulement de l'anesthésie, certains actes opératoires, les effets secondaires postopératoires possibles et les signes et seuils d'alertes à surveiller. Ils évoquaient l'importance du rôle de coordonnateur du médecin traitant en amont comme en aval. Ils évoquaient le rôle primordial des aidants, dont l'aide était perçue comme indispensable au moment de la sortie ainsi qu'au domicile pour la gestion des actes de la vie quotidienne. La réalisation de certains soins par leurs propres moyens et la délégation de la surveillance médicale aux aidants étaient vécues pour certains comme un « transfert de responsabilité ». Conclusion : L'évolution sociétale et des techniques médicales, la volonté du maintien des dépenses de santé sont à l'origine du développement de la chirurgie ambulatoire. Le renforcement de la coordination entre les professionnels de santé de soins primaires et hospitaliers est primordial afin de garantir, en toute sécurité pour le patient, une prise en charge optimale. La vigilance est de mise afin que le médico-économique ne prenne pas le pas sur le médico-social.

L'accompagnement du parcours de soins du patient fait partie des compétences du médecin généraliste, renforcées par la loi HPST et la stratégie Nationale de Santé. L'objectif principal de cette étude était d'explorer les représentations de la coordination des soins ville- hôpital chez les professionnels de santé du service de chirurgie ambulatoire (CMCA), de l'hôpital Huriez, au CHRU de Lille. Méthode : Une étude qualitative utilisant une approche par théorie ancrée a été conduite, à l'aide d'entretiens semi-dirigés collectifs et individuels. L'analyse des données a été effectuée après retranscription puis double codage informatique des données à l'aide du logiciel Nvivo®. Résultats : la chirurgie ambulatoire semble concerner des actes restreints chez des patients à l'état de santé connu. Cette éligibilité est permise à la condition d'une coordination de bonne qualité avec les professionnels de santé de soins primaires. La programmation et la protocolisation de la prise en charge des patients permettent d'anticiper et d'éviter les aléas. Cette coordination des soins est satisfaisante pour la plupart des médecins et chirurgiens interviewés mais perfectible en interne. Améliorer la communication au sein de l'hôpital permettrait une meilleure coordination des soins en extrahospitalier. Conclusion : Une confrontation des représentations de la coordination des soins par les différents acteurs de santé concernés est une étape préalable à la réalisation d'une étude d'intervention. Des pistes de modifications pourront alors émerger.
