La qualité microbiologique de l’eau à la sortie des units dentaires fait l’objet de nombreuses recommandations. Leur conception favorise la stagnation de l’eau et par conséquent la prolifération d’une flore hydrique et la formation de biofilms. Ainsi, la question du risque de transmission d’agents infectieux pathogènes au patient et à l’équipe soignante se pose. Quels sont les équipements et des traitements utilisés dans les hôpitaux parisiens ? Sont-ils efficaces vis à vis des normes en vigueur ? Est-ce suffisant ? Ce travail se divise en deux parties et débute par une revue de littérature permettant de décrire le risque infectieux lié à l’eau des units. La seconde partie est dédiée à l’analyse quantitative de la flore revivivifiable et des agents pathogènes à la sortie des units dentaires du service d’odontologie de l’hôpital Rothschild. L’objectif de cette étude était de contrôler la qualité microbiologique de l’eau à la sortie de ces units. Des prélèvements d’eau ont été effectués et envoyés dans deux laboratoires indépendants. 24 prélèvements ont été analysés sur 12 units dentaires d’âge variable. Les résultats révèlent que 7 units répondent aux recommandations du guide technique de l’eau dans les établissements de santé pour les critères « eau pour soins standards ». Sur les 5 autres, les seuils de flore aérobie revivifiable à 22°C et 36°C sont largement atteints pour les deux plus anciens. Trois units neufs présentent des non-conformités à leur mise en place. Du Pseudomonas aeruginosa, micro-organisme pathogène, est détecté sur 33 % des units étudiés. Suite à ces contaminations, une désinfection complète des circuits a été entreprise par la société qui entretient les units dentaires, selon une procédure dite cadrée, validée par le chef de service. Cette thèse met en évidence la difficulté d’enrayer le problème de gestion des risques liés à l’eau de soins. Des guides de bonnes pratiques et des recommandations rappellent l’importance de maîtriser la qualité microbiologique de l’eau.
