Objectif. Évaluer les motivations, l'utilisation et l'acceptabilité d'une contraception masculine thermique (CMT) dans une cohorte historique d'hommes utilisant la CMT comme unique contraceptif de couple. Matériel et méthodes. Nous avons sollicité 72 hommes ayant commencé à utiliser la CMT entre 2011 et 2019, pour une enquête anonyme en ligne (93 questions) explorant les caractéristiques de la population, les antécédents contraceptifs, les motivations à choisir la CMT, l'expérience des patients aux différentes phases de la méthode, les relations avec partenaire et l'environnement, et la satisfaction de cette méthode. Résultats. Soixante-sept hommes ont répondu (taux de réponse de 93%), 63 ont été inclus et ont rempli le sondage (taux de participation de 94%). Les principales motivations du choix de la CMT étaient « l'absence d'hormones » (n=59/63, 94%) et l'utilisation d'une méthode considérée comme « naturelle » (n=49/63, 78%). La plupart des hommes (68%) se sont approprié la méthode en moins de 2 semaines. La demande croissante de partager la charge contraceptive dans les couples et l'absence de contraception masculine efficace disponible sur le marché nous ont amené à rechercher les données de la science sur les méthodes de contraception masculine thermique (CMT). Nous avons réalisé une revue systématique de la littérature, qui a permis d'identifier 14 articles d'études interventionnelles publiées entre 1965 et 2019. Leur objectif était d' étudier l'inhibition de la spermatogenèse chez des hommes euspermiques par élévation de la température testiculaire (ETT) de faible intensité. Certaines études ont aussi étudié sa réversibilité, son efficacité contraceptive et son son innocuité. La méthode d'ETT majoritairement étudiée était l'utilisation d'un dispositif surélevant les testicules en position suprascrotale pendant 15 ou 24 heures par jour. Une inhibition de la spermatogenèse partielle à totale, avec altération de la quantité et de la qualité des spermatozoïdes, a été observée par toutes les études sauf une. La réversibilité de l'inhibition a été satisfaisante à l'arrêt de l'exposition. L'efficacité contraceptive était étudiée par 3 études, sur une durée cumulée de 512 mois, avec l'absence de grossesse pour tous les couples ayant utilisé la méthode de CMT sans interruption. La durée maximale d'utilisation contraceptive a été de 47 mois. Aucun effet secondaire grave n'a été rapporté pour les hommes exposés. Des anomalies du noyau des spermatozoïdes observées pendant l'exposition ont été réversibles à l'arrêt. Les données publiées confirment l'efficacité contraceptive de la CMT par l'utilisation d'un dispositif de surélévation testiculaire en position suprascrotale. Des études de plus grande ampleur sont nécessaires pour conforter ces résultats en population générale. Une commercialisation comme dispositif médical et une communication plus large sur cette méthode sont nécessaires à une plus grande accessibilité d'utilisation. La formation des professionnels de santé de soins primaires (médecins généralistes, sages-femmes) ainsi que leur implication dans des études à plus grande échelle participeraient grandement à la diffusion de la CMT.

Introduction : La contraception est à ce jour essentiellement un sujet féminin. Face à une demande importante, des associations françaises d'usagers développent des méthodes contraceptives masculines. Deux méthodes inhibant la spermatogenèse, développées dans les années 1980, font actuellement leur retour dans l'arsenal contraceptif : la contraception masculine hormonale et la contraception masculine thermique (CMT). L'objectif principal est de décrire la population nantaise utilisant la CMT, de connaître ses pratiques et ses connaissances des recommandations de L'AFU. L'objectif secondaire est d'évaluer l'efficacité d'inhibition de la spermatogenèse par la CMT chez nos patients. Matériel et Méthodes : Une étude descriptive rétrospective a été menée du 01/01/2018 au 25/06/2022 au CHU de Nantes, 59 patients utilisant la CMT ont répondu au questionnaire en ligne. Il a également été évalué par une étude rétrospective l'efficacité contraceptive de la CMT au CHU de Nantes, 32 patients avec au moins un spermogramme avant mise en place de la CMT et un spermogramme de contrôle d'efficacité ont été inclus. Résultats : Le questionnaire décrit la population comme jeune (25-30 ans), en couple non marié et sans enfant. La méthode contraceptive principalement utilisée est l'anneau Andro-switch à 90 % et le début de la CMT est très récent (2021 ou 2022). Les recommandations sont globalement respectées, exceptées sur la fréquence des spermogrammes de suivi une fois le seuil atteint. L'avis global est très positif aussi bien pour l'utilisateur que pour l'entourage. In response to persistent donor organ shortages, organs from marginal donors, such as expanded criteria donors (ECD) and donation after cardiac death (DCD) donors, are now accepted and have been successfully transplanted, reducing the waiting times for transplantation. Especially in Japan, transplantation of DCD kidneys has a relatively long history because of the difficulty or lack of national consensus in accepting brain death, which has made it possible to accumulate considerable clinical experience.
