This book discusses the results of an international project about new ways of organising health information. The project Chance – Community Health Management to Enhance Behaviour – based on the preliminary assumption that a local community’s structure determines the perception of health information and health promotion in a specific way. Therefore the relevance of space in dailylife was the starting point for an interdisciplinary research in six European countries with very different frames of health systems. As it is obvious that knowledge of a healthy lifestyle is one essential factor for its implementation, the multiple offers of health information and education have their justification. But, the most effective offers are those that relate to a local community’s specific needs. This book shows different ways to install tailored local projects. The main important keywords are participation, local networks and social cohesion. The idea of a “Community Health Management” outlined in the last chapter, is to change the perspective of health work. The project was funded in the EU Lifelong Learning Programme Grundtvig over two years. Here are presented the main results for stakeholders, students and researchers.

With European health systems facing a plethora of challenges that can be addressed through public health interventions there is renewed interest in strengthening public health services. Yet there are enormous gaps in our knowledge. How many people work in public health? How much money is spent on public health? What does it actually achieve? None of these questions can be answered easily. This volume brings together current knowledge on the organization and financing of public health services in Europe. It is based on country reports on the organization and financing of public health services in nine European countries and an in-depth analysis of the involvement of public health services in addressing three contemporary public health challenges (alcohol obesity and antimicrobial resistance). The focus is on four core dimensions of public health services: organization financing the public health workforce and quality assurance. Demographic, epidemiological, social and cultural trends in European countries are changing the traditional patterns of care. The next decades will see rising numbers of care-dependent older people and noncommunicable diseases as the leading cause of chronic illness and disability. Further, urbanization and the break-up of the traditional large family group will lead to gaps in the care of older or disabled family members. These changes in needs and social structure require a different approach to policy and services in the health and social sectors; a disease-oriented approach alone is no longer appropriate. Home care could be an answer to these issues: a sustainable approach to prevent the need for unnecessary acute or long-term institutionalization and to maintain people in their homes and communities as long as possible. Along with new forms of service delivery organization, technological innovation can represent a viable solution to developing home care in Europe, provided that health care systems can further enhance integration and coordination. This publication is part of the work of the WHO Regional Office for Europe to present evidence for health policy- and decision-makers in a clear and understandable form. It explains why health and social services should provide high-quality and targeted home care for disabled and older people.
