This book provides readers with a critical, conceptual and applied understanding of the role of communication and community engagement for disease outbreak preparedness and response. Until the WHO declared COVID-19 a pandemic on March 11, 2020, for several years public health authorities and influential voices in the international public health community have warned of a pandemic and therefore a need to strengthen governments and communities’ ability to prevent and respond to it effectively to minimize its impact on lives and economies. While investments have focused on clinical, diagnostic, and vaccine research, preventing and minimizing the impact of disease outbreaks requires a wider socio-ecological systems approach that places communities at the centre of the response. Such an approach is still rare in public health practice. One of the key lessons that the authors have learned, and on which they reflect in the chapters, is that technical inputs will be as effective as they are fully integrated within the broader architecture of disease outbreak preparedness and response. The ten chapters of this contributed volume are organized under three parts: a conceptual framework, case studies, and recommendations. Communication and Community Engagement in Disease Outbreaks is a timely and essential resource for public health managers, donors, implementers, organizations engaged in disease prevention and control and academics called on to support the response. Building communication capacity is a critical piece of preparing for, detecting, and responding to infectious disease threats. The International Health Regulations (IHR) establish risk communicationâ€"the real-time exchange of information, advice, and opinions between experts or officials and people who face a threat to their survival, health, and economic or social well-beingâ€"as a core capacity that World Health Organization member states must fulfill to strengthen the fight against these threats. Despite global recognition of the importance of complying with IHR, 67 percent of signatory countries report themselves as not compliant. By investing in communication capacity, public health and government officials and civil society organizations facing health crises would be prepared to provide advice, information, and reassurance to the public as well as to rapidly develop messages and community engagement activities that are coordinated and take into account social and behavioral dynamics among all sectors. To learn about current national and international efforts to develop the capacity to communicate effectively during times of infectious disease outbreaks, and to explore gaps in the research agenda that may help address communication needs to advance the field, the Forum on Microbial Threats of the National Academies of Sciences, Engineering, and Medicine convened a 1.5 day workshop on December 13 and 14, 2016, in Washington, DC. Participants reviewed progress and needs in strengthening communication capacity for dealing with infectious disease threats for both outbreaks and routine challenges in the United States and abroad. This publication summarizes the presentations and discussions from the workshop.

Drawing on a growing consensus about the importance of community representation and participation for ethical research, community engagement has become a central component of scientific research, policy-making, ethical review, and technology design. The diversity of actors involved in large-scale global health research collaborations and the broader ‘background conditions’ of global inequality and injustice that frame the field have led some researchers, funders, and policy-makers to conclude that community engagement is nothing less than a moral imperative in global health research.
