In the 1930s, during the authoritarian government of Getúlio Vargas, the Brazilian civil service reform movement began. Thirty-five years later, the actual administrative practices of the country did not adequately reflect the philosophy underlying this movement, a philosophy drawn from the reform experience and public administration theories of the United States and Western Europe. This book examines why these ideas, when transplanted to another cultural setting, did not take root and, further, why they unexpectedly proved to be most applicable in Brazil during periods of autocratic rule. These questions are highly relevant not only to Brazil, but equally to other developing countries struggling to create more effective national administrative systems. For this reason, and in order to evaluate the Brazilian reform experience within its total context (social, economic, and political), Lawrence S. Graham develops a broad conceptual framework. His focus is on the years between 1945 and 1964, a period which allowed a relatively free play of political forces but, ironically, produced a diminution in the success of the reform efforts when compared with the authoritarian governments which preceded and followed it. After a comparative consideration of the public administration theories behind the reform movement, Graham examines this period in terms of the political environment, the functions of political patronage, and the influences of a nascent national party. Finally, he juxtaposes the conditions and course of the Brazilian reform experience with those of the United States and Great Britain. Graham’s study of the Brazilian example, which does not pass judgment on the prevailing public personnel system, reveals the importance of understanding the total cultural context within which administrative principles are put into practice. Such an approach, wider than generally held in the field of public administration, may prove to be the most vital factor in the future of the civil service in Brazil and several other countries facing the same problems.

It has been more than 20 years since Brazil's 1988 Constitution formally established the Unified Health System (Sistema Unico de Saude, SUS). Building on reforms that started in the 1980s, the SUS represented a significant break with the past, establishing health care as a fundamental right and duty of the state and initiating a process of fundamentally transforming Brazil's health system to achieve this goal. This report aims to answer two main questions. First is have the SUS reforms transformed the health system as envisaged 20 years ago? Second, have the reforms led to improvements with regard to access to services, financial protection, and health outcomes? In addressing these questions, the report revisits ground covered in previous assessments, but also brings to bear additional or more recent data and places Brazil's health system in an international context. The report shows that the health system reforms can be credited with significant achievements. The report points to some promising directions for health system reforms that will allow Brazil to continue building on the achievements made to date. Although it is possible to reach some broad conclusions, there are many gaps and caveats in the story. A secondary aim of the report is to consider how some of these gaps can be filled through improved monitoring of health system performance and future research. The introduction presents a short review of the history of the SUS, describes the core principles that underpinned the reform, and offers a brief description of the evaluation framework used in the report. Brazil's pension system takes up an oversized proportion of its social protection spending. It comprises of Regime Geral de Previdéncia Social (RGPS), covering private sector workers, and over two thousand Regimes Pröprios de Previdéncia Social (RPPS), insuring public civil servants at federal and subnational levels. While the total membership of RPPS only stands at about 10 percent of RGPS coverage, its spending amounts to almost half of RGPS pension outlays. This paper attempts to present an integrated view of RPPS pension schemes, their influence on subnational budgets, and their interaction with human resource policies. After a brief introduction, Chapter 3 starts by documenting the history of civil service and its associated pension schemes, looking for explanations on how subnational RPPS became so big, dispersed, and difficult to reform. The fiscal consequences of subnational civil service pension scheme expansion and maturation, including RPPS role in the fiscal challenges and policies of the last few years, are discussed in Chapter 4. Chapter 5 attempts to expose important interlinkages between pension and human resource policies and argues for the need of integrated policy approach. Chapter 6 describes the history of previous RPPS reform attempts, while Chapter 7 focuses on the effects of federal pension reform of 2019 on subnational civil servant pension schemes. The paper ends with lessons and policy recommendations for the future.

Social security is the single most important fiscal issue facing the Brazilian government today. This report summarizes the state, and potential policy implications, of the Brazilian Social security system. It also discusses policy recommendations for: social security and pensions, the national social security system, government pensions and funds, and the complementary pension systems. An overview of the social security challenge reviews the system components, revealing unsustainable fiscal imbalances and administrative weaknesses in both the unreformed General Regime for Social Security (RGPS), and the Pension Regime for Government Workers (RJU), with large tax-related distortions, and labor market inefficiency. Thus the goals of Brazil's reforms are to reduce fiscal deficits, lower actuarial imbalances, increase equity and redistribution, reduce collateral inefficiencies, and facilitate growth of funded pensions. The study implies there is no recourse for the country, but to lower the high, uniform replacement rates (experience suggests that rates higher than 40-70 percent, cannot be sustained). The key to effective reform of social security is widening the debate to include potential winners from these changes, particularly the private sector, the young, and the poor. Policy recommendations suggest that the adverse equity effects of RGPS reforms should be widely publicized to generate political support for deeper RJU reform.
