This is the fourth volume of our series Progress in Anti-cancer Therapy. For the past four years we have taken the challenge to select each year, some of the most interesting topics on the wide field of oncology. As usual, this volume continues the tradition and covers five cancers (breast, prostate, bladder, lym phoma and ovarian), the role of HIV and HTLV in cancer, as well as some deve lopmental pharmacology and behavioral issues. This year, we have particularly focused our attention on one of the most com mon (although rarely addressed) cancer accounting for more than 315,00 newly diagnosed cases and 41,000 cancer-related deaths annually (Landis SH et al., 1999) : prostate cancer. Five chapters address some of the issues concerning this disease. The chap ter by Schroder describes the hopes and pitfalls of early detection of prostate cancer. It reviews the literature on screening studies that have been performed both in the US and in Europe. These studies suggest that cancer mortality could be reduced by screening for prostate cancer, mostly through PSA testing. However, the effectiveness of early detection still needs to be definitely confir med and ongoing randomized studies are described, the results of which will probably help the medical community determine the worth of PSA-based scree ning for prostate cancer. On the same hand, the paper from von Eschenbach is reviewing the clinical problems that are related this disease.

Chemotherapy for-cancer is in a state of evolution. Because some cancers can now be cured with chemotherapy as a singular modality, this therapy can no longer be viewed as simply a palliative contribution. Chemotherapy has assumed an important role as an adjuvant to other modalities, including both surgery and radiation therapy. For some tumors, the primary application of chemotherapy in a combined modality approach to curative therapy has resulted in the application of less radical surgery while achieving substantial of relatively rare tumors such as cure rates. Nonetheless, with the exception childhood tumors, hematologic malignancies, and testicular cancer, the effec tiveness of chemotherapy in most tumors is severely limited. At the cellular level, greater understanding of the specific mechanism of tumor cell killing and of the phenomenon of drug resistance are elusive, critical ingredients in the improvement of effectiveness in cancer chemotherapy. Prolonging the exposure time of the tumor cell to drugs is a concept that was addressed in the early phases of the development of chemotherapy. How ever, technological limitations inhibited the broader application of chemother apy by infusion until recent years. Also, the convenience of intermittent ther apy on an outpatient basis, with the predominant theory of drug effect based on a dose response as well as the proven effectiveness of this schedule in some tumors, has slowed the process of subjecting infusion chemotherapy to the rigors of clinical trials.

Cisplatin, the first member of the family of platinum-containing chemotherapeutic agents, was discovered by Barnett Rosenberg in 1965 and approved by the FDA for marketing in 1978. After 30 years of use in the clinic, cisplatin remains a central element of many treatment regimens. Cisplatin is still an irreplaceable component of a regimen that produces high cure rates in even advanced nonseminomatous germ-cell cancers, and is widely used in the treatment of ovarian cancers and other gynecologic cancers, head and neck, and numerous other tumor types. The development of carboplatin has reduced some of the adverse events associated with cisplatin treatment, and the introduction of the DACH platinum compound oxaliplatin has broadened the spectrum of activity of the platinums to include gastro-intestinal cancers, especially colorectal cancer. The clinical importance of this family of drugs continues to drive investigation into how these drugs work and how to improve their efficacy and reduce their toxicity. The papers in this volume were presented in Verona, Italy, during the tenth International Symposium on Platinum Coordination Compounds in Cancer Chemotherapy. The symposium was jointly organized by the Department of Oncology of the Mater Salutis Hospital – Azienda Sanitaria Locale 21 of the Veneto Region – and by the Department of Medicine and Public Health, Section of Pharmacology, the University of Verona. They reflect the vitality of this field and the increasing use of new molecular and cell biologic, genetic, and biochemical tools to identify approaches to further improve their use.
