The role of dissociation as a mediator of childhood sexual abuse and emotional and behavioral mental health outcomes was investigated. The sample consisted of 114 children and adolescents (ages 11--18 years) who were wards of the Illinois Department of Children and Family Services (DCFS) and currently living in state-supervised residential treatment centers, apart from their families. This study assessed the relationship between varied histories of childhood abuse (sexual abuse, physical abuse, combined physical and sexual abuse), dissociative responses, and a variety of psychiatric outcomes using a multimethod design. Children were interviewed directly using several structured assessment tools, and their present caregivers (i.e., residential staff) were asked to complete questionnaires. A retrospective chart review was also used as an additional perspective on children's history and current functioning. Results indicated that sexual abuse was significantly associated with dissociation compared to other types of abuse history. Both sexual abuse and dissociation were independently associated with a several indicators of emotional and behavioral disturbance and risk-taking behaviors. Finally, dissociation was frequently a significant covariate for sexual abuse and psychiatric status. For instance, dissociation was a significant covariate for one measure of overall symptomatic functioning; previously significant main effects for sexual abuse disappeared after controlling for dissociation. These results were replicated across several assessment sources and different perspectives. Therefore, dissociation appears to serve as significant mediator function for emotional and behavioral sequelae among sexually abused children. The potential importance of more widespread assessment of dissociation among childhood populations and the implications of these findings are discussed.
