Notre étude a eu pour but d'observer les patients présentant un syndrome coronaire aigu avec sus-décalage du segment ST qui ont échappé à la filière pré-hospitalière recommandée et pris en charge aux urgences du CHU de Toulouse, d'avril 2015 à avril 2016. La prévalence estimée aux urgences est de 10 SCA ST+ pur 10000 dont 7 évoluant depuis moins de 12 heures. 67 patients ont été inclus dans notre étude, avec 67,3 ans de moyenne d'âge, et 39% de femmes. L'hypertension artérielle est le premier facteur de risque retrouvé (42%) suivi du tabagisme (30%) et des antécédents personnels d'accident ischémique aigu (22%). Les femmes ont un profil plus à risque que les hommes avec une hypertension artérielle dans un cas sur deux et un tabagisme actif dans un cas sur deux. Les femmes jeunes présentent de surplus un antécédent familial d'accident ischémique (45,5%). Trois patients sur quatre avaient appelé le Centre d'appel du SAMU (15). La proportion de patients pris en charge par le SMUR est de 65%, en baisse depuis 2008. Le délai médian de premier contact médical a été de 128 minutes (66 - 227) plus faible à Purpan (60, 23 - 166) qu'à Rangueil (144, 79 - 230). Les taux de traitement adjuvant et de transfert en service de cardiologie interventionnelle sont conformes aux recommandations (91% et 89,6%). Le délai cible de transfert en cardiologie de 60 minutes fixé par l'HAS semble dépassé pour plus de la moitié des patients. Des axes d'amélioration de la reconnaissance de ces patients en régulation sont à rechercher

Introduction : on constate une dégradation de l'efficacité de la filière de soins du SCA ST+ en Haute-Garonne. Matériel et méthode : étude épidémiologique observationnelle, du 1er avril 2015 au 31 janvier 2017. Nous avons récupéré les données de régulation de tous les patients pris en charge aux urgences du CHU de Toulouse, ayant un diagnostic final de SCA ST+ et ayant appelé le SAMU 31 avant l'admission. Résultats : le profil type du patient " oublié de la filière SCA ST+ " était : une femme de plus de 75 ans, ayant une douleur thoracique atypique évoluant depuis moins de 3 heures, une douleur persistante à l'arrivée des secours, à 6/10 d'EVN. Elle décrivait 1 à 2 symptômes associés et présentait 1 à 2 facteurs de risque cardiovasculaire. Conclusion : il faudrait organiser une nouvelle formation des médecins régulateurs avec évaluation de son efficacité sur la prise en charge locale.
