Actuellement, les modes d'investissement professionnels, aussi bien des femmes que des hommes médecins tendent vers un juste équilibre entre l'organisation de leur activité professionnelle et la gestion de leur vie familiale. Les nouvelles générations de médecins redoutent que l'installation en milieu rural ne permette pas cet équilibre. Apporter une contribution à la connaissance des déterminants de la décision de jeunes MG ruraux à s'installer et à se sentir bien dans leur travail. Recherche qualitative par entretiens semi-dirigés auprès de MG installés en milieu rural dans le Puy-de-Dôme et de professionnels ressources. Les facteurs favorisants l'installation rurale étaient l'expérience des remplacements, le regroupement, le soutien des patients et l'accompagnement du projet d'installation. Les freins étaient représentés par la crainte de perdre leur qualité de vie et les difficultés de gestion d'un cabinet. Le milieu rural se concevait à condition de l'avoir préalablement expérimenté et si l'entourage familial adhérait au projet, souvent dans le but de répondre aux besoins d'un territoire déficitaire. La charge de travail et l'éloignement étaient les principales contraintes évoquées propres au milieu rural. Enfin, l'accès au bien-être dépendait des possibilités d'une maitrise de sa charge de travail, d'une organisation optimale du cabinet médical, et du sentiment d'être un bon médecin, permises par l'exercice coordonné. Parvenir en milieu rural à réguler sa charge de travail et préserver sa qualité de vie ne s'improvise pas. Il est nécessaire de s'y confronter au préalable, notamment par l'exercice de remplacements. Si beaucoup de contraintes imposées par le territoire rural dépendent de paramètres solutionnables, le concept d'attachement au territoire (inné ou acquis) est ancré. L'accès au bien-être au travail en milieu rural semble pour la plupart des jeunes MG passer par l'exercice coordonné. Le concept nouveau « d'équipe de soin primaire » semble profitable à la fois aux MG et aux patients.

Introduction : La pénurie médicale en zone rurale est un problème majeur de santé publique. Des mesures sont prises pour favoriser l'installation médicale, afin d'améliorer l'offre de soin sur tout le territoire. L'objectif de ce travail est d'étudier les facteurs déterminants à l'installation rurale de médecins généralistes dans le secteur bas-normand. Méthode : Une étude qualitative a été menée auprès de médecins généralistes par le biais d'entretiens semi-dirigés et individuels, suivis d'une analyse thématique continue et manuelle des données. Résultats : L'analyse de 10 entretiens a mis en évidence plusieurs facteurs déterminants. L'organisation adaptée du lieu de travail et l'orientation vers un travail d'équipe étaient primordiales. La pratique globale, tant par la diversité d'actes que par la relation privilégiée avec la patientèle, représentait un déterminant majeur. La découverte du métier par le biais de stages universitaires et de remplacements paraissait indispensable de façon précoce durant les études. L'origine géographique des médecins ainsi qu'un attrait pour la vie en milieu rural impactaient directement le choix d'installation future. Les aides financières étaient considérées comme importantes, mais pas déterminantes à l'installation. Conclusion : Les facteurs déterminants à l'installation de médecins généralistes en zone rurale sont nombreux. Il semble intéressant de multiplier les terrains de stages ambulatoires de médecine générale en milieu rural afin de permettre à tous les étudiants de découvrir cette pratique singulière et de les sensibiliser à la ruralité. Aussi, il semble nécessaire d'évaluer d'autres types d'aides pour pérenniser l'offre de soin.

It is now widely recognized that human resources for health (HRH) are a key enabler for the attainment of universal health coverage, and for the achievement of Sustainable Development Goal (SDG) 3 - Ensure healthy lives and promote well-being for all at all ages. As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, therefore, enables governments and other relevant stakeholders to adopt a holistic, rather than fragmented, approach to ensuring that the health workforce contributes both to improved health and to broader socioeconomic development.

Manthia Diawara provides an insider's account of the history and current status of African cinema. African Cinema: Politics and Culture is the first extended study in English of Sub-Saharan cinema. Employing an interdisciplinary approach which draws on history, political science, economics, and cultural studies, Diawara discusses such issues as film production and distribution, and film aesthetics from the colonial period to the present.
