The aim of this project is to extend accurate and patient-specific treatment planning to new treatment modalities, such as molecular targeted radiation therapy, incorporating previously crafted and proven Monte Carlo and deterministic computation methods. A flexible software environment is being created that allows planning radiation treatment for these new modalities and combining different forms of radiation treatment with consideration of biological effects. The system uses common input interfaces, medical image sets for definition of patient geometry, and dose reporting protocols. Previously, the Idaho National Engineering and Environmental Laboratory (INEEL), Montana State University (MSU), and Lawrence Livermore National Laboratory (LLNL) had accrued experience in the development and application of Monte Carlo-based, three-dimensional, computational dosimetry and treatment planning tools for radiotherapy in several specialized areas. In particular, INEEL and MSU have developed computational dosimetry systems for neutron radiotherapy and neutron capture therapy, while LLNL has developed the PEREGRINE computational system for external beam photon-electron therapy. Building on that experience, the INEEL and MSU are developing the MINERVA (Modality Inclusive Environment for Radiotherapeutic Variable Analysis) software system as a general framework for computational dosimetry and treatment planning for a variety of emerging forms of radiotherapy. In collaboration with this development, LLNL has extended its PEREGRINE code to accommodate internal sources for molecular targeted radiotherapy (MTR), and has interfaced it with the plug-in architecture of MINERVA. Results from the extended PEREGRINE code have been compared to published data from other codes, and found to be in general agreement (EGS4 - 2%, MCNP - 10%)(Descalle et al. 2003). The code is currently being benchmarked against experimental data. The interpatient variability of the drug pharmacokinetics in MTR can only be properly accounted for by image-based, patient-specific treatment planning as has been common in external beam radiation therapy for many years. MINERVA offers 3D Monte Carlo based MTR treatment planning as its first integrated operational capability. The new MINERVA system will ultimately incorporate capabilities for a comprehensive list of radiation therapies. In progress are modules for external beam photon-electron therapy and Boron Neutron Capture Therapy (BNCT). Brachytherapy and Protontherapy are planned. Through the open Application Programming Interface (API) other groups can add their own modules and share them with the community.

Monte Carlo simulations have been applied to determine and study different parameters that are challenged in experimental measurements, due to its capability in simulating the radiation transport with a probability distribution to interact with electrosferic electrons and some cases with the nucleus from an arbitrary material, which such particle track or history can carry out physical quantities providing data from a studied or investigating quantities. For this reason, simulation codes, based on Monte Carlo, have been proposed. The codes currently available are MNCP, EGSnrc, Geant, FLUKA, PENELOPE, as well as GAMOS and TOPAS. These simulation codes have become a tool for dose and dose distributions, essentially, but also for other applications such as design clinical, tool for commissioning of an accelerator linear, shielding, radiation protection, some radiobiologic aspect, treatment planning systems, prediction of data from results of simulation scenarios. In this chapter will be present some applications for radiotherapy procedures with use, specifically, megavoltage x-rays and electrons beams, in scenarios with homogeneous and anatomical phantoms for determining dose, dose distribution, as well dosimetric parameters through the PENELOPE and TOPAS code.

A fast and accurate treatment planning system is essential for radiation therapy and Monte Carlo (MC) techniques produce the most accurate results for dose calculation in treatment planning. In this work, we developed a fast Monte Carlo code based on pre-calculated data (PMC, Pre-calculated Monte Carlo) for applications in radiation therapy treatment planning. The PMC code takes advantage of large available memory in current computer hardware for extensive generation of pre-calculated data. Primary tracks of electrons are generated in the middle of homogeneous materials (water, air, bone, lung) and with energies between 0.2 and 18 MeV using the EGSnrc code. Secondary electrons are not transported but their position, energy, charge and direction are saved and used as a primary particle. Based on medium type and incident electron energy, a track is selected from the pre-calculated set. The performance of the method is tested in various homogeneous and heterogeneous configurations and the results were generally within 2% compared to EGSnrc but with a 40-60 times speed improvement. Pre-calculated Monte Carlo codes are accurate, fast and physics-independent and therefore applicable to different radiation types including heavy-charged particles. In another project, we worked on Monte Carlo feasibility study to use orthogonal bremsstrahlung beams for imaging in radiation therapy. The basic characteristics of orthogonal bremsstrahlung beams are studied and the feasibility of improved contrast imaging in linear accelerator with such a beam is evaluated. In the context of this work orthogonal bremsstrahlung beams represent the component of the bremsstrahlung distribution perpend.

The McGill Monte Carlo treatment planning system (MMCTP), provides a flexible software environment to integrate Monte Carlo simulations with current and new treatment modalities.
