Le nombre de maladies infectieuses émergentes a quintuplé en 40 ans. A l'ère de la mondialisation, le caractère pandémique des zoonoses devient attendu. La pandémie de SARS-COV-2 de 2020 a illustré cette observation. Le plan national de lutte créé en 2009 et révisé en 2011 est principalement axé sur la pandémie grippale en lien avec la transmission aviaire et porcine à l'humain. Il n'a pas été créé de plan systémique pour que le système de santé puisse faire face à une pandémie de plus grande ampleur. La hausse de mortalité liée au COVID-19 sur l'année 2020 a concerné surtout les personnes de 65 ans et plus. La gestion de la crise sanitaire dans les cabinets de médecine générale a été faite dans l'urgence. Un effet sur le suivi des patients atteints de pathologies chroniques (que nous désignerons par "PAPC" dans le reste de cet ouvrage) était donc prévisible. L'objectif de ce travail était double. Décrire l'adaptation des pratiques d'une manière générale, et focaliser sur le suivi des PAPC. Etude qualitative par focus group menée par deux chercheurs. Nous avons interrogé des praticiens de médecine générale sur les adaptations de suivi de leurs patients chroniques lors de la pandémie COVID-19. Analyse en deux étapes : déductive basée sur la méthode SEPO puis inductive par théorie ancrée. Trois focus groups ont permis d'atteindre une convergence des occurrences lors du codage des verbatims. Des succès, échecs, potentialité et des obstacles à l'adaptation des médecins généralistes pendant la crise COVID-19 ont été mis en évidence. Des facteurs facilitateurs et des freins au suivi des patients chroniques pendant cette crise ont été identifiés. L'analyse des résultats met en évidence trois niveaux d'adaptation influençant le suivi des PAPC lors de la crise COVID-19. Au niveau individuel, la surcharge de travail entrave fortement les capacités d'adaptation des praticiens. Au niveau professionnel, les praticiens se sont sentis isolés et le mode de rémunération actuel a été questionné. Au niveau sociétal, le rôle du médecin généraliste et les risques encourus lors de leur pratique n'ont pas été anticipés. La pandémie COVID-19 nous a rappelé les conséquences sanitaires des échanges mondialisés. Dans un système économique et sanitaire global inchangé, de futures pandémies sont attendues. Leur gestion au niveau national peut être anticipée en prenant en compte les trois niveaux décrits qui sont interdépendants.
