The nutritional status of children is declining as evidenced by the steady rise in childhood obesity rates over the last three decades. Consuming five servings of fruit and vegetables (FV) daily has shown to help with weight maintenance, but children are consuming far less than the recommended servings. This study was designed to test the efficacy of a sensory-focused multi-component school-based program at increasing vegetable intake among fifth-graders. Classroom, family, and community components allowed children to explore thirteen target vegetables with their senses including taste. Vegetable consumption was measured by digitalized observations of lunchtime vegetable selection and consumption. Vegetable acceptance was evaluated using a self-administered survey assessing attitude and behaviors related to vegetable consumption. Measures were assessed at multiple time points and compared between the intervention school and a comparison school matched for demographic similarities. Parental consent was obtained for 136 fifth-graders to participate in the multicomponent study and 114 were included in the plate waste study. Data were collected over six days of plate waste observations including two phases: the control phase (CP) and the target vegetable phase (TVP) where target vegetables were served in addition to the regular lunch vegetables. Due to nutrition transition, the prevalence of childhood obesity is increasing significantly worldwide especially in affluent developing countries such as Kuwait. Obesity is associated with adverse chronic health conditions and financial burdens which provides an incentive for prevention. The objective of the study was to examine the effectiveness of a multi-component school-based nutrition intervention designed to improve healthy eating knowledge, attitudes, self efficacy and dietary behaviors among fourth and fifth graders in Kuwait. The Let's Eat Smart program was based on the social cognitive theory and used short interactive lessons, DVD sessions, posters, food models and parental letters to promote a healthy lifestyle message among fourth and fifth graders. Let's Eat Smart was delivered to 110 students in the English Playgroup and Primary School. Surveys and 3-day food-records were collected on pre- and post-intervention. Surveys assessed nutrition knowledge, attitudes, dietary behaviors and self efficacy while diet records were analyzed to examined intake of food groups. Abstract: Child health promotion is a salient public health goal. Childhood obesity rates have risen dramatically over the past few decades with more than 20% of youths overweight or obese by the time they enter middle school. Population-based strategies including those delivered through schools are needed to positively impact this trend. The IMOVE program evaluation, a quasi-experimental study involving one intervention and one comparison school, examines the influence of a school-based healthy eating program in a middle school cafeteria serving low-income, racially-diverse adolescents. Study 1 examined the association between the availability of healthy lunch meals (IMOVE meals) in school cafeterias and lunch and snack food purchase patterns of students. The introduction of IMOVE resulted in significantly less participation in purchase of low nutritional quality snack foods from fall to spring term. Students in both schools had similarly high participation in school lunch all year long, suggesting that IMOVE participation was sustainable. Study 2 examined the association between body mass index and purchase of IMOVE and standard school lunch meals in the intervention school. This study also identified sociodemographic predictors of participation in the school lunch program. Students who were overweight or obese had a significantly higher rate of purchase of both types of lunches, but even more so for standard school lunch than for IMOVE meals, compared to students who were not overweight/obese. Other characteristics significantly associated with participation in IMOVE and standard school lunch were nonwhite race, sixth grade, and low-income status. Study 3 examined the association between exposure to IMOVE and total daily food and nutrient intake measured using an abbreviated food screener before and after the intervention. At follow-up, students in the IMOVE school consumed sugary beverages and higher-fat milk offerings less often in their daily diets than students in the comparison school. Efforts to combat childhood obesity through changes in the school food environment have some impact, but require the support of policy action and nutrition education initiatives to be most effective. The IMOVE program proved feasible, acceptable, and sustainable, providing a model upon which to build additional intervention components to more comprehensively impact student wellness.

Most youth in the US do not meet national recommendations for nutrient-dense foods such as fruits and vegetables. School-based interventions can contribute to promotion of these foods from a young age, as youth spend a large proportion of their day and consume one or more meals while at school. Multicomponent, school-based interventions are recommended as a strategy for promoting fruit and vegetable consumption, but there is wide variation in program design and approaches to evaluation. This includes the use of a range of methods for measuring dietary outcomes, many of which have unknown or weak validity. Further development and application of objective, observation-based dietary assessment methods may strengthen this evidence base. To address this gap, this dissertation focused on the application and adaptation of an observation-based dietary assessment method, digital imaging, to enable more rigorous yet feasible measurement of diet in the context of school-based interventions. The objective of the first study within this dissertation was to examine the impact of a multicomponent, school-based intervention on lunchtime fruit and vegetable dietary behaviors, by applying digital imaging. Using a randomized, controlled design as part of a pilot study involving fourth-grade students from two elementary schools, this study examined changes in lunchtime dietary behaviors before and after the introduction of the Shaping Healthy Choices Program. This program was designed to integrate nutrition education, family and community partnerships, and improvements to school food availability. Changes in school lunch intake were compared between intervention and control students for fruit (n = 111) and vegetable (n = 112) outcomes using exact tests and t-tests. FAO provides countries with technical support to conduct nutrition assessments, in particular to build the evidence base required for countries to achieve commitments made at the Second International Conference on Nutrition (ICN2) and under the 2016-2025 UN Decade of Action on Nutrition. Such concrete evidence can only derive from precise and valid measures of what people eat and drink. There is a wide range of dietary assessment methods available to measure food and nutrient intakes (expressed as energy insufficiency, diet quality and food patterns etc.) in diet and nutrition surveys, in impact surveys, and in monitoring and evaluation. Differenct indicators can be selected according to a study's objectives, sample population, costs and required precision. In low capacity settings, a number of other issues should be considered (e.g. availability of food composition tables, cultural and community specific issues, such as intra-household distribution of foods and eating from shared plates, etc.). This manual aims to signpost for the users the best way to measure food and nutrient intakes and to enhance their understanding of the key features, strengths and limitations of various methods. It also highlights a number of common methodological considerations involved in the selection process. Target audience comprises of individuals (policy-makers, programme managers, educators, health professionals including dietitians and nutritionists, field workers and researchers) involved in national surveys, programme planning and monitoring and evaluation in low capacity settings, as well as those in charge of knowledge brokering for policy-making.
