Personal protective equipment (PPE) is critical to the protection of healthcare workers responding to infectious disease outbreaks. The ability of the PPE supply chain to provide adequate and consistent supply when there is a large spike in demand has not been well-considered. Humanitarian logistics literature rarely considers infectious disease outbreaks as possible humanitarian crises while epidemiology literature assumes perfectly responsive supply chains. This thesis uses a mixed methods approach - an exploratory case study and system dynamics model - to bridge the gap between these two fields. It provides one approach for connecting epidemiology and supply chain research. An explanatory case study of the 2014 West Africa Ebola outbreak is used to analyze the PPE supply chain and its in-crisis functionality. We gather primary data using semi-structured interviews with supply chain actors and analyze that data using qualitative coding analysis. The system dynamics model is developed based on the results of the case study to offer insight as to how the PPE supply chain could be improved to better respond to future outbreaks. Several scenarios are simulated to test the effects of various supply chain improvement strategies. Relationship-building between supply chain actors, unconstrained shipping channels, flexible funding pools, and pre-positioning are all found to be effective supply chain improvement strategies.

These downfalls resulted in detrimental human and economic loss across the entire nation. No state has been spared, and the United States' decision to act as fifty separate states, rather than one unified nation, has only aggravated the problem. At the end of this thesis, an analysis will be performed on New Zealand, a country that responded successfully to the pandemic through a universal and united strategy. This thesis will begin with an overall look into the federal and state governments' roles in responding to national emergencies, as well as an overview on healthcare supply chain management in the United States. The thesis will then delve into the supply chain operations behind personal protective equipment, both before and during the pandemic. Next, the thesis will take a quick look at New Zealand's overall response to COVID-19, in order to consider key discrepancies between the response in the United States. Lastly, the thesis will conclude with recommendations on how to better improve the United States' response and outlook on the supply chain behind personal protective equipment. After careful analysis of the current state of emergency preparedness and supply chain management surrounding personal protective equipment, it is clear that the United States must make the necessary improvements to ensure the protection of the economy and health of the citizens. This analysis highlights the need for a more universal approach to emergency preparation and response, establishing a plan for an increase in domestic manufacturing of personal protective equipment, and shifting away from the just-in-time healthcare delivery system.

When communities face complex public health emergencies, state local, tribal, and territorial public health agencies must make difficult decisions regarding how to effectively respond. The public health emergency preparedness and response (PHEPR) system, with its multifaceted mission to prevent, protect against, quickly respond to, and recover from public health emergencies, is inherently complex and encompasses policies, organizations, and programs. Since the events of September 11, 2001, the United States has invested billions of dollars and immeasurable amounts of human capital to develop and enhance public health emergency preparedness and infrastructure to respond to a wide range of public health threats, including infectious diseases, natural disasters, and chemical, biological, radiological, and nuclear events. Despite the investments in research and the growing body of empirical literature on a range of preparedness and response capabilities and functions, there has been no national-level, comprehensive review and grading of evidence for public health emergency preparedness and response practices comparable to those utilized in medicine and other public health fields. Evidence-Based Practice for Public Health Emergency Preparedness and Response reviews the state of the evidence on PHEPR practices and the improvements necessary to move the field forward and to strengthen the PHEPR system. This publication evaluates PHEPR evidence to understand the balance of benefits and harms of PHEPR practices, with a focus on four main areas of PHEPR: engagement with and training of community-based partners to improve the outcomes of at-risk populations after public health emergencies; activation of a public health emergency operations center; communication of public health alerts and guidance to technical audiences during a public health emergency; and implementation of quarantine to reduce the spread of contagious illness.

In response to the COVID-19 outbreak, many governments recommended or mandated the use of personal protective equipment (PPE) in public and private settings to prevent the spread of the virus, resulting in a sharp surge in demand. However, PPE manufacturers were not able to increase production fast enough to meet demands. For this reason, many govern-ments worldwide failed to secure enough PPE to protect their frontline workers in hospitals and clinics. This study aimed to compare the procurement, production, and distribu-tion challenges that PPE manufacturers, distributors, and governments faced during the 2020 PPE shortage and the strategies that governments in China, Germany, and the United States applied to secure their PPE supply. The purpose of this study was further to derive recommendations for public decision makers to secure medical supplies during health emergencies sim-ilar to COVID-19. An exploratory case study of these three countries was conducted based on 12 semi-structured interviews with supply chain management and healthcare experts, PPE manufacturers and distributors, and one govern-ment representative between September and October 2020. Additionally, secondary data from newspaper articles, governments, press releases, and websites were used to enrich the research through data triangulation. This study's findings revealed many challenges, such as fierce competition, the time needed for acquiring new suppliers, lack of workforce, raw mate-rials, and machines, limited transportation capacities, and export re-strictions, making it challenging to secure enough PPE. Additionally, the results show that the observed governments actively engaged in the healthcare supply chain by financing, securing, and distributing PPE while pursuing different strategies. The conducted research contributes to the discussion on the disaster man-agement literature by presenting challenges and strategies that emerged in this crisis. Furthermore, it provides recommendations for public decision makers to improve their preparedness and responsiveness for future health emergencies.
