Introduction : En France, en 2016, 218 600 personnes étaient prises en charge pour des maladies inflammatoires chroniques de l'intestin. Ces pathologies, source de symptômes invalidants, altèrent la qualité de vie des malades. En le rendant acteur de sa prise en charge, l'éducation thérapeutique permet au patient de mieux gérer sa maladie au quotidien. Dans le cadre de la procédure de renouvellement de ces programmes, l'Agence Régionale de Santé demande des évaluations régulières. Selon la haute autorité de santé, les données qualitatives sont sous-exploitées dans ces évaluations. Objectif : Analyser le vécu de l'éducation thérapeutique par les patients ayant bénéficié du programme Nice MICI. Méthode : étude qualitative par entretiens semi-dirigés auprès des patients suivis pour une MICI au CHU de Nice. L'analyse thématique a été précédée d'un double codage. Résultats : Onze patients ont été interrogés. Les informations délivrées par les professionnels de santé et leur écoute bienveillante participaient à l'établissement d'un partenariat soignant-soigné de qualité. Le patient évoluait dans une atmosphère rassurante, propice à la discussion et au partage d'expériences entre patients, permettant de lutter contre l'isolement. Le programme apportait une aide au patient dans la gestion de son traitement, de son environnement et de son alimentation. Enfin, il lui était enseigné comment réagir face à diverses situations du quotidien. Mais la surinformation actuellement disponible rend le patient plus critique à l'égard de la prise en charge, notamment diététique. Plusieurs patients ont également évoqué le recours à certaines thérapies complémentaires pour pallier les différents symptômes. Conclusion : En le rendant acteur de sa prise en charge, le programme permet au patient d'acquérir les compétences d'auto-soins et d'adaptation relationnelle, nécessaires pour vivre au mieux avec sa maladie. Néanmoins, l'analyse du discours met en évidence l'émergence de nouvelles croyances et pratiques que le patient n'est pas toujours en mesure de critiquer justement. Il est illusoire de vouloir lutter contre ces pratiques. Ainsi, les professionnels de santé doivent rester alertes et ouverts aux différentes propositions thérapeutiques qui circulent afin de guider au mieux le patient.

Les Maladies inflammatoires chroniques de l'intestin (MICI) regroupent deux maladies différenciées que sont la maladie de Crohn et la rectocolite hémorragique. Un siècle après la découverte de ces maladies, leur prise en charge est encore en pleine évolution, avec un arsenal thérapeutique de plus en plus varié. L'accompagnement des patients atteints de maladies chroniques est essentiel. La prise en charge médicale doit pouvoir inclure cet accompagnement aussi bien à l'hôpital par le biais de l'éducation thérapeutique du patient (ETP), qu'en pharmacie d'officine, à travers les conseils et informations donnés par le pharmacien, professionnel de santé de proximité du patient. Il n'existe pas de traitement permettant la guérison de la maladie, mais un contrôle de la fréquence, durée et de l'intensité des poussées. La prise en charge thérapeutique a beaucoup évolué ces dernières années : des aminosalicylés, aux biothérapies. Ces dernières ont révolutionné la prise en charge des MICI. Les MICI font l'objet de nombreuses prescriptions en officine. En raison de l'arrivée de nouvelles classes médicamenteuses renforçant l'arsenal thérapeutique, il est nécessaire que l'ensemble des professionnels de santé au contact de ces patients puissent délivrer les conseils associés à la délivrance de ces médicaments et répondre aux questions des patients. Aujourd'hui, la pharmacie d'officine doit intégrer une partie de pharmacie clinique à ses fonctions, et créer un lien entre la ville et l'hôpital afin de prendre en charge au mieux les patients atteints d'une maladie chronique. Il est essentiel pour la prise en charge du patient, qu'il se sente accompagné en ville comme à l'hôpital. Un accompagnement pluridisciplinaire devrait exister pour l'ensemble des patients.
