Le gynécologue-obstétricien, le gynécologue médical libéral, le médecin généraliste et la sage-femme sont habilités à suivre la grossesse en fonction de risque. Les politiques de périnatalité sont aussi présentes dans le suivi de grossesse pour un meilleur accès à la préparation à la naissance et à la parentalité. Ainsi, différents types de trajectoires s'offrent aux femmes primipares avec une action plus ou moins active dans le choix de l'offre de soins. Quatorze entretiens semi-directifs de femmes primipares d'une zone rurale rencontrées en postpartum ont permis de comprendre quelles étaient leurs attentes, leur choix et le vécu du suivi de leur grossesse par ces différents professionnels. Quels que soient les choix propres à chaque professionnel, les parturientes recherchent une relation de confiance et de disponibilité. Elles ont globalement exprimé un avis favorable sur le suivi de grossesse. Chaque professionnel, en partenariat, doit continuer à suivre la grossesse en fonction du risque mais en les informant mieux sur l'offre de soins et la préparation à la naissance et la parentalité. Enfin suite au départ à la retraite de médecins gynécologues et généralistes, dans cette zone rurale déficitaire médicalement, la sage-femme apparaît alors comme le professionnel de recours pour suivre plus de grossesses physiologiques.

Objectifs : Le but de ce mémoire est d’évaluer le vécu des femmes devenant mères à 40 ans ou plus, sur le plan physique et psychologique, en y conjuguant l’analyse de l’impact de leur suivi médical et du comportement de l’entourage social et familial. Méthode : Une étude descriptive rétrospective a été réalisée auprès de 52 primipares agées d’au moins 40 ans ayant accouché entre le 1er janvier 2010 et le 31 décembre 2010 dans les maternités des Centres Hospitaliers d’Antibes Juan-Les-Pins et de Grasse. Résultats : Majoritairement actives et en couples, les femmes de notre panel ont, pour 56% d’entre elles, eu une grossesse physiologique, leur suivi médical ayant été majoritairement assuré par un médecin. Seul un tiers des patientes a beneficié d’un entretien prénatal précoce et 7l% ont suivi les séances de préparation à la naissance. En postnatal, 28% ont bénéficié d’un accompagnement à domicile, par une sage-femme dans la majorité des cas. La majorité a qualifié sa grossesse d’épanouissante même si 1/3 l’a vécue comme source de stress. L’entourage a été plutôt soutenant, le père ayant été tres présent et investi en périnatal. Enfin, même si 67% des patientes ont considéré leur suivi médical comme rassurant, un tiers l’a jugé hypermedicalisé. Discussion et conclusion: Le vécu reste tres variable d’une femme à l’autre et dépend notamment de l’attitude des professionnels de santé au cours de la grossesse, dont le discours informatif ne doit pas nuire. Ces couples doivent pouvoir béneficier d’une prise en charge personnalisée le plus tôt possible, la sage-femme étant référent unique, au sein d’un dispositif pluridisciplinaire centré autour d’eux.

Described by some as a “necropolis for babies,” the province of Quebec in the early twentieth century recorded infant mortality rates, particularly among French-speaking Catholics, that were among the highest in the Western world. This “bleeding of the nation” gave birth to a vast movement for child welfare that paved the way for a medicalization of childbearing. In Babies for the Nation, basing her analysis on extensive documentary research and more than fifty interviews with mothers, Denyse Baillargeon sets out to understand how doctors were able to convince women to consult them, and why mothers chose to follow their advice.
