Objectif : l'objectif de cette étude est de décrire les transferts aux urgences des résidents en EHPAD. Matériel et méthode : Il s'agit d'une étude longitudinale prospective et descriptive du dossier de transfert aux urgences, des modalités de ces transferts et des patients venant d'EHPAD sur une période de 3 mois, allant du 5 février au 5 mai 2014. Résultats : la quasi-totalité des résidents d'EHPAD admis aux urgences le sont avec un dossier de transfert. Plus de 96% d'entre eux comportent les antécédents et le traitement. Le motif du recours et l'histoire de la maladie sont présents pour respectivement 92% et 77% des dossiers. 47% des dossiers stipulent le GIR. La personne de confiance est mentionnée dans 27% des cas. Les directives anticipées sont transmises dans moins de 3% des cas. 1% des dossiers précisent des prescriptions anticipées. Les patients sont âgés de 87,7 ans en moyenne, 83% d'entre eux sont GIR 1-3 et 55% sont déments. Ils sont admis à 70% en journée, 40% à la demande du médecin traitant pour des motifs à 46% cardiorespiratoires. Une infirmière est présente à l'EHPAD au moment du transfert dans 71% des cas. 45% des patients ne présentent aucune défaillance clinique à l'admission, 36% retrouvent leur structure sans être hospitalisés. Une décision de limitation thérapeutique est prise pour 16% des patients et 8% décèdent à l'hôpital. Conclusion : le Dossier de Liaison d'Urgence doit être amélioré et des mesures peuvent être prises en EHPAD pour tenter de réduire les transferts inappropriés aux urgences.
