My personal interest in cardiac Dopplerarose from longexperience in diagnos ing carotid artery disease using Doppler continuous wave ultrasound with fre quencey spectral analysis. The work of Dr. Liv Hatle of Trondheim, who was then using analogue tracings for cardiac Doppler, inspired me to try CW Doppler combined with spectral analysis and high-pass filtering techniques. The results with this system, without benefit of standard echocardiography were so pro ductive that a special instrument was produced by Carolina Medical Electronics for worldwide marketing. Our experience led to the development of an interna tional symposium and training program. These symposia gave birth to the Inter national Cardiac Doppler Society and Volume I of this book series. This second volume on Cardiac Doppler Diagnosis is an outgrowth of the successofVolume I and the excellent work of many clinical scientists. Doppler instrumentation continues to improve with integration ofboth contin uous wave and pulsed wave Doppler with echocardiography. Color directional Doppler imaging techniques led byourJapanese colleagues is finding acceptance as a rapid screening method to find special regions of interest where more quantitative continuous wave with its quantitative capabilities of Doppler can be brought to bear. I wish to thank the many fine authors ofthese two volumes for their excellent efforts and, again, Marian Wienker for her prodigious assistance.

Ultrasound diagnostic techniques comprise an important element of this new field. Dr. Arabin has taken the initiative to investigate the functional-diagnostic aspects of ultrasound. Among other things, she has further developed and refined the concept of "oxygen-con serving adaptation of fetal circulation" which originated in the Department of Obstetrics and Gynecology at the Neukolln Hospital Center in 1966. She thus has been able to show that the most reliable Doppler blood flow meas urement predictors of a high risk to the fetus are (1) a decrease in the flow volume of the descending thoracic aorta and the umbilical artery and (2) an increase in the flow volume of the common carotid arteries.

Echocardiology comprises all aspects of diagnostic application of ultrasound to cardiac patients. It is probably the fastest growing non-invasive technique today. Almost all progress in this young and exciting field has been the positive result of close co-operation between medical and technical scientists. In the 1940's the only diagnostic aids were the electrocardiogram and simple X-ray. These were quickly followed by the cardiac ca theter, phonocardiography, radio isotope methods and angiocardiography. The de velopment of cardiac surgery provided the impetus to developing more accurate methods of diagnosis, preferably those that did not need invasion of the patient. The introduction of ultrasound has contributed towards this aim in the last few years.
