The Committee's report examines the causes of deaths in custody, and considers what may be done to prevent these deaths, and better protect the right to life and other human rights, of vulnerable people held in the custody of the state. Issues discussed include: human rights standards applicable under the European Convention on Human Rights; the scale of the problem and concerns relating to the wider penal system in which these deaths occur, including the issue of overcrowding in prisons and sentencing practice; risk assessment and management, including reception in police custody, immigration detention and the provision of physical and mental healthcare in detention; the use of physical restraint and seclusion; staffing and training issues; investigations into deaths in custody and inquiries. Recommendations include the establishment of a cross-departmental expert task-force on deaths in custody to monitor the topic, review good practice standards, publish information and to make recommendations to Government.

The Major Projects Report 2003 provides information on progress made by the Ministry of Defence in procuring major defence equipment against cost, time and technical performance targets. It covers 30 projects split, according to Smart Acquisition principles, between the 20 largest projects on which the main investment decision has been taken, and the 10 largest projects yet to reach that point. For the 20 largest projects, costs are forecast to have increased by £3.1 billion in the last year, and are now 6.1 per cent over their approved costs. Difficulties on four older projects (which predate the introduction of Smart Acquisition) account for the majority of cost and time overruns. Following on from a NAO report on this topic (HCP 195, session 2003-04; ISBN 0102926581) published in January 2004, the Committee's report examines four main issues: the impact of the large cost overruns and delays; departmental risk management; ways of developing a more constructive relationship between the Department and industry; and lessons that need to be learned to avoid such poor procurement performance being repeated in future.

The Committee's report examines continuing concerns about the performance of the Child Support Agency (CSA), particularly in relation to its compliance and enforcement regime. Issues discussed include: the old and new child support schemes; performance targets and information management; the IT problems encountered; child support assessments; compliance targets and debt levels; enforcement measures in the UK and other countries, notably Australia; staffing issues including staff training, job cuts, pay and morale; and options for urgent action and longer term reforms. The CSA is found to be a failing organisation which is currently in crisis, and requires radical reform in order to provide an acceptable service. Senior management has failed to provide effective leadership through significant cultural shift, with deficient training given to frontline staff and inadequate monitoring procedures in place. The Committee strongly recommends that proposed job cuts be suspended until the Agency's business transformation programme, including its IT system, is proved to be fully functioning. If the recommended improvements are not made, the CSA may need to be wound up and alternative policies implemented to provide financial support and maintenance for children.

The Committee's report examines the rehabilitation of offenders, looking in particular at the effectiveness of prisons in reducing re-offending through rehabilitation, given the problems of overcrowded conditions. The Committee is happy with the effectiveness of the Electoral Fraud (Northern Ireland) Act 2002 but there are still flaws in the current arrangements. Less than 84 percent of these eligible to vote in Northern Ireland, are currently registered; young people and socially economically depressed groups are especially under-represented. Less than 25 percent of 17 and 18 years olds are registered. Voter's names are no longer placed automatically on next year's register, which is an effective anti-fraud measure, but on the other hand this has highlighted the low level of registration. Fewer of the new Electoral Identity Cards (EIDs) were issued than expected, and, while other forms of photographic ID are acceptable at polling stations,over 3,500 voters were turned away from polling stations at recent elections, because of ID problems. An example of the lack of joined-up thinking was the poor performance of two competing Government telephone help lines during the 2003 canvass period.
