Head and Neda - The pupils are sluggish, medium Wide, and react to light and accommodation. The mouth shows marked pyorrhea and a heavily coated tongue, yellowish in color, with red tip and edges. The neck shows one or two moderately enlarged glands. The ears, nose, and scalp are negative. The thorax is symmetric; expansion is equal on both sides and excursion is good. The lungs are entirely negative. No titles can be heard and no areas of dulness can be made out. In particular, nothing abnormal was elicited at the apices. T he heart - The apex-beat is in the fifth interspace; no murmurs and no thrills can be heard. The heart sounds are rapid, ranging from 114 to 130. This examination was made on the morning following his admission, at a time, as you will see from the temperature-curve, when he had developed considerable fever. The abdomen is a little bit tense, but not definitely rigid. The liver dulness shows nothing special; the liver is not pal pahle. There is a little tenderness in the suprapubic region. Excerpt from The Medical Clinics of Chicago, Vol. 1: November, 1915; Number 3 I saw a great many more cases of typhoid as an intern, in Cincinnati, than I do now, and I made up my mind that when I got through my apprenticeship I would do something besides feed patients on milk alone. Little by little physicians became more liberal in their diet, but the majority of men up to five or six years ago kept their patients principally upon milk, giving them perhaps other uids. This was a little better, but during the last four or five years the fashion has swung around the other way. Men are recommending food in quantities - a good deal more than the ordinary diet contains. A man will recommend as much as 4000 or 4500, or even 5000 calories of food a day, which is as much as a man doing the hardest kind of work can consume. I have always opposed the simple milk diet and the simple uid diet, but I am just as much against the overfeeding. I see no reason why a patient that is running a fever should be fed more than he would if he were up and about. I have never put a patient on a pure uid diet, but I don't believe in overfeeding either. In the first place, I think a diet consisting of about 2800 or 3000 calories is just about right. That is just about what most of us consume in a day. Is it necessary to keep the typhoid patients on a liquid diet? Emphatically No. The idea was that by keeping the patient on a strictly milk diet the fecal residue would be less, but that is not true. In the various examples of stasis or cardiac insufficiency which you have seen clinically you may observe that the essential symptomatology pivots on the myocardium. Conventionally, we classify heart diseases as those of the pericardium, myocardium, and endocardium - besides those of nervous disturbance. Practically, from the therapeutic standpoint, most cardiac affections are, on last analysis, myocardial. Valvular diseases, sharply distinguished from each other by their peculiar physical findings, attract attention first when the overstrained heart muscle becomes more or less "incompetent." The fresh effusion of pericarditis causes other than local signs and symptoms chiefly when the fluid presses upon the heart and obstructs its complete diastole, and the later adhesions impede the systole by anchoring the heart to the chest wall and mediastinum.
