Excerpt from The Journal of the Oklahoma State Medical Association, 1916, Vol. 9 Any transitory cause producing on the vascular system increased blood pressure may be spoken of as functional hypertension. The increase of tension due to excessive mental activity or physical exertion, as found m forcible inspiration in normal man, are physiological acts which force a large amount of blood to the heart and brain. Increase In the viscosity of the blood impedes the circulation of the blood througho the capillaries (russell), and hence high tension. Acute gastric and intestinal pain may cause hypertension (curschmann.) Nicotine, alcohol, ergot, adrenalin, pituitrin, hydrastine hydrochlorate and 'stypticin are among the drugs which cause increase in blood-pressure. Nicotine (and pyridine) is one of the most destructive agencies to constructive metabolism in the arteries of the brain and should be ranked with alcohol.' Toxic factors are found in eclampsia (h. Vasquez), gout, alimentary intoxication, bact'eremias and uremia. Psychical hypertension of great intensity is seen in fright, anger, sudden joy and great sorrow. Extreme high tension may accompany acute compression of the brain ln frac ture of the base of the skull, and m apoplexy. Acute cerebral anemia produces the same symptoms as cerebral compression, hence, high tension. Obstruction of the cerebral sinuses and veins may be due to pressure 011 the innominate or jugular veins, by a tumor or aneurysm, to suffocation and strangling, to excessive strain, to tricuspid insufficiency, to embolism, to thrombosis, to arterial and venous degeneration, to ulceration, to abscess, and to hemorrhage. Wein burger observed ln the case of a gardener of 36, an aneurysm and rupture of the vessels, and the basal arteries and aorta were sound. Excerpt from The Journal of the Kansas Medical Society, Vol. 18: January 1918 to December 1918, Inclusive Comment - The origin of this case is uncertain. One of the horses driven from Colorado developed distemper after arriv ing in Neosho County, but recovered. There were no open sores nor glandular enlarge ment according to the statement of the father. The other horse was healthy. The children brought a pet rabbit from Colo rado. And this, also. Appeared to be in a healthy condition. The only explanation of the origin of the infection that seems plausible is that the patient became ih fected at one of the numerous camping places along the route. This case, to my mind, is typical of sporotrichosis, in this country, and is at tended with several interesting features. First, the age of the patient, she being four years old; there having been but two younger reported, one by Quain. Age three years, and the other by Ruediger and Smith. Age three and a half years, both females. Second, that no treatment or household remedies had been applied pre vions to my seeing the case. Third, the clean condition of the initial lesion, no mixed infection. Fourth, smears made from the lesion demonstrated the organism. True nephritis is concerned with the filter portion of the kidney, and the failure to filter out all the bacteria and their retention is responsible for the occurrence of one form of the disease, which is of great surgical importance. As to just how often such infections may be ascending or lymphogenous rather than hematogenous in origin there is a diversity of opinion. My own opinion is that such infections, other than hematogenous, are extremely rare so far as the kidney filter is concerned. This permits a large finger to reach the postero lateral angle of the diseased bone, and gives room for the introduction of the curette, sharp spoon, or forceps, and leaves a large space for a drainage-tube. Should it be desired to carry the drainage-tube through the column, the trans verse process with the end of the rib of the op posits side can also be removed, when it can readily be passed through.
