These notes were developed to assist in the review of the basic science of pathology for those students preparing to sit for various national proficiency examinations which include, as part of their content, material dealing with general and systemic pathology. As such, the notes are not intended to replace the many fine textbooks of pathology whose scope, detail, and mission are more suited to the in depth study of pathology. Rather, these notes are intended to serve as "memory joggers" to rekindle and refresh the mind of some of the salient features of various disease processes and to point out to the student possible areas of weakness which they may need to address in more detail by referring to one of the standard textbooks of pathology. Fabry disease is an X-linked inborn error of metabolism wherein deficiency of a lysosomal enzyme results in systemic deposition of glycosphingolipids. Storage deposition, and hence pathological disease, occurs preferentially in renal glomerular and tubular epithelial cells, myocardial cells, heart valve fibrocytes, neurons of dorsal root ganglia, and in endothelial smooth muscle cells of blood vessels. Thus, Fabry disease is a multi-system disorder, albeit with considerable phenotypic heterogeneity in onset and in severity; however, it is progressive, exhibits extensive morbidity, and is life-threatening. Within the past two decades, there has been a radical change in the natural course Fabry disease by virtue of the availability of specific enzyme replacement therapy.
