Abstract:Background: Double crowns have served as attachments for remaining healthy natural teeth and periodontally compromised dentition. Additionally, they have fine long-term clinical outcomes when used in implant-supported overdentures. However, the treatment concept of telescoping implant fixed dentures for edentulous patients has been described only in few case reports so far. Our team believes telescopes can be flexibly used in retaining full-mouth implant-supported fixed protheses. Purpose: The aim of this investigation was to figure out the implant and prosthetic survival rates, the healthy condition of peri-implant soft and hard tissues, patient satisfaction and the incidence rates of prosthetic maintenance for an average follow-up period of 3 years. Material and Methods: From March 2012 to June 2017, 17 edentulous patients (11 male and 6 female; mean age 56) were selected to have implant-supported fixed telescopic prostheses. A total of 18 protheses were fabricated(8 maxillary and 10 mandibular). Fixed dentures retained by telescopic crowns were used as the final prosthesis, with milling titanium or all-ceramic abutments serving as primary crowns and gold-electroforming crowns serving as secondary crowns. Health status of peri-implant soft tissue, marginal bone loss, patient satisfaction and prosthetic complications were evaluated at the time of follow-up visits (3 months, 1 year, 2 years and 3 years after definitive rehabilitation). Data was counted and analyzed applying SPSS v23 software package. Quantitative data about health condition of soft-tissue and bone loss during three-yearu2019s observation time was evaluated by the method of multilevel analysis. Multilevel analysis was performed by means of R version 3.1.0. Results: All 17 patients (56 u00b1 6.1 years/range: 42.7-68.5) were treated with one-piece implant-supported fixed telescopic dentures. There were 58 implants in the maxilla and 53 implants in the mandible. 109 implants out of 111 implants were retained, yielding a 98.2% cumulative implant success rate. Radiography results indicated stable marginal bone loss in survival implants. There was no significant differences in modified plaque index and modified sulcus bleeding index over this time (P>0.05). The probing attachment level deteriorated by 1.5 mm during the first 3 years (P0.05). Among the restorations, 2 porcelain fractures were detected but neither had notable impact on the restoration outcomes. All 18 restorations provided sufficient fixation and stability and all patients were satisfied with the outcomes. Totally 2 porcelain fractures occurred but had no influence on the function of restoration. The frequency of prosthetic maintenance per patient per year was 0.11. Conclusion: One-piece implant-supported fixed detachable telescopic restoration is an effective method with short-term esthetic and stable outcomes in edentulous patients. One of the key benefits is the access of both retrievability and retentive force. Besides, it provides the possibility of restoring an edentulous mouth with fixed dentures using implants even situated in undesirable positions or direction.
