Introduction : historiquement les sténoses du tronc commun gauche (TCG) étaient revascularisées par pontage aorto-coronarien (PAC).Les premières études évaluant la revascularisation par angioplastie retrouvaient une surmortalité par rapport au PAC. Avec le développement des stents actifs, l'évolution des techniques d'angioplastie et des antiagrégants plaquettaires, les données actuelles indiquent que l'angioplastie du tronc commun est une alternative au PAC. Les syndromes coronariens aigus (SCA) secondaires à une sténose TCG sont peu fréquents mais associés à un pronostic sombre. La revascularisation urgente par angioplastie est devenue le gold standard dans le SCA avec sus décalage ST ou dans l'état de choc cardiogénique. Le mode de revascularisation dans le SCA sans sus ST stabilisé est plus discuté notamment pour les résultats à long terme. Peu de données récentes sont disponibles sur les résultats à court et à long terme de la revascularisation par angioplastie chez ces patients. L'objectif de cette étude est d'évaluer les résultats de l'angioplastie du TCG à court et long terme dans une population tout venant de patients hospitalisés pour SCA (avec ou sans sus-décalage du segment ST). Méthodologie : 134 patients ont été inclus entre janvier 2014 et décembre 2018. Le critère de jugement principal était la mortalité cardiovasculaire (CV). Le critère de jugement secondaire était un critère composite d'évènements cardiovasculaires majeurs (MACE) comprenant les décès cardiovasculaires, les revascularisations répétées au site d'angioplastie initial, les récidives de SCA, les réhospitalisations pour insuffisance cardiaque, les AVC ischémiques et les hémorragies graves. Résultats : la population était majoritairement composée d'hommes (72,4%) d'âge moyen de 72+/-12 ans. 28 patients (20,8%) ont présenté un décès CV à un suivi moyen de 4+/-0,2 ans, 20 durant la phase hospitalière et 8 durant le suivi post-hospitalier. En analyse multivariée, l'Euroscore 2 (HR : 1,1 ; 95% CI (1,03-1,14) ; p=0.001) et le taux de lactate à l'admission (HR : 1,22 ; 95% CI (1,07-1,39) ; p=0.003) étaient prédicteurs de décès cardiovasculaire intra-hospitalier. As with the introduction of x -ray computed tomography, much of the initial development of magnetic resonance applications tended to focus on the central nervous system. The development of magnetic resonance imaging applications to other organ systems such as the chest, abdomen, pelvis and extremities has lagged somewhat behind, awaiting technical improvements, and a broader user base.
