Les études ont démontré la faisabilité et l'intérêt d'un sevrage des hypnotiques chez la personne âgée, avec un effet bénéfique sur les risques de chute, la somnolence diurne, les troubles cognitifs, la qualité de vie. Cependant il n'existe pas d'étude sur le sevrage chez le patient dément, qui représente une part importante des résidents d'EHPAD. Cette efficacité repose sur la mise en route de stratégie de diminution des doses, que l'on peut appliquer aux résidents de ces institutions. L'efficacité est néanmoins plus importante quand on y adjoint le respect de mesures environnementales et la réalisation de thérapies cognitives et comportementales, qui semblent difficiles à appliquer dans les EHPAD du fait de leur mode de fonctionnement et de leurs contraintes. Les outils d'aide au médecin généraliste existent pour la prise en charge d'une insomnie et la réalisation d'un sevrage par diminution de dose, mais ils font défaut pour l'aide à la mise en œuvre des méthodes non médicamenteuses. Une véritable politique de réduction de la prescription médicamenteuse doit comporter une réflexion sur l'organisation des soins en EHPAD afin de permettre la mise en œuvre de mesures environnementales ou comportementales améliorant la qualité du sommeil.
