In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

In November 2017, the The Forum on Promoting Children's Cognitive, Affective, and Behavioral Health, in collaboration with the Roundtable on the Promotion of Health Equity, convened a workshop on promoting children's behavioral health equity. The workshop used a socio-ecological developmental model to explore health equity of children and families, including those with complex needs and chronic conditions. Particular attention was paid to challenges experienced by children and families in both rural and urban contexts, to include but not limited to poverty, individual and institutional racism, low-resourced communities, and hindered access to educational and health care services. Workshop participants also engaged in solution-oriented discussions of initiatives, policies, and programs that aim to improve social determinants of health, opportunities for behavioral health promotion, and access to quality services that address the behavioral health of all children and families. This publication summarizes the presentations and discussion of the event.

The World Health Organization defines the social determinants of health as "the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life." These forces and systems include economic policies, development agendas, cultural and social norms, social policies, and political systems. In an era of pronounced human migration, changing demographics, and growing financial gaps between rich and poor, a fundamental understanding of how the conditions and circumstances in which individuals and populations exist affect mental and physical health is imperative. Educating health professionals about the social determinants of health generates awareness among those professionals about the potential root causes of ill health and the importance of addressing them in and with communities, contributing to more effective strategies for improving health and health care for underserved individuals, communities, and populations. Recently, the National Academies of Sciences, Engineering, and Medicine convened a workshop to develop a high-level framework for such health professional education. A Framework for Educating Health Professionals to Address the Social Determinants of Health also puts forth a conceptual model for the framework's use with the goal of helping stakeholder groups envision ways in which organizations, education, and communities can come together to address health inequalities.

By providing information on other sector's agendas, policy approaches and their health impacts, and by illustrating areas for potential collaboration, the Social Determinants of Health Sectoral Briefing Series encourages more systematic dialogue and problem solving and more collaboration with other areas of government. Examples of intersectoral action for health--current and historical--reveal that health practitioners are frequently perceived as ignoring other sectors' goals and challenges. This creates barriers to intersectoral work, limiting its sustainability and expansion. In order to avoid this perception, instead of starting from the goals of the health system (e.g. health, health equity, responsiveness, fairness in financial contributions), the Social Determinants of Health Sectoral Briefing Series focuses on the goals of other sectors. Rather than concentrating on traditional public health interventions (e.g. treatment, prevention, protection), the series uses the goals of other sectors to orient its analyses and explore areas of mutual interest. The target audience for the series is public health officers who are not experts on determinants of health, but who have responsibilities for dealing with a broad range of development issues and partners. Each briefing will focus on a specific policy area, summarizing and synthesizing knowledge from key informants in health and other areas, as well as from the literature. It will present arguments, and highlight evidence of impacts and interventions, with special emphasis on health equity. It will make the case to health authorities for more proactive and systematic engagement with other sectors to ensure more responsive and cohesive governments that will meet broader societal aspirations for health, equity and human development.

Evidence indicates that actions within four main themes (early child development fair employment and decent work social protection and the living environment) are likely to have the greatest impact on the social determinants of health and health inequities. A systematic search and analysis of recommendations and policy guidelines from intergovernmental organizations and international bodies identified practical policy options for action on social determinants within these four themes. Policy options focused on early childhood education and care; child poverty; investment strategies for an inclusive economy; active labour market programmes; working conditions; social cash transfers; affordable housing; and planning and regulatory mechanisms to improve air quality and mitigate climate change. Applying combinations of these policy options alongside effective governance for health equity should enable WHO European Region Member States to reduce health inequities and synergize efforts to achieve the United Nations Sustainable Development Goals.

The decade ahead will test the nation's nearly 4 million nurses in new and complex ways. Nurses live and work at the intersection of health, education, and communities. Nurses work in a wide array of settings and practice at a range of professional levels. "If you ask most people what influences health, almost invariably the first reply will be health care-the services that individuals receive from physicians, nurses, and other medical professionals to treat or prevent illness, also called medical care. Many people, in fact, presume the role of health care (medical care) in affecting health to be so predominant that they often use the terms health and health care interchangeably. Many people would probably also cite behaviors such as diet, exercise, smoking, and use of alcohol or drugs as key influences on health. While ample evidence supports the importance of both health care and behaviors for health, a compelling body of scientific knowledge now calls for a wider and deeper set of explanations for why some of us experience good health and others do not. This body of knowledge challenges us to think beyond common assumptions about the key causes of health and illness, to ask not only "What influences health?" but also "What factors shape those influences?" i.e., "What influences the influences?" This knowledge tells us that, to achieve real and lasting improvements in health, we must shift the focus to identifying and addressing the root or fundamental causes(Link and Phelan 1995)-the underlying factors that set in motion other factors that may be more easily observed but play a less fundamental role in shaping health. Ethical concerns, furthermore, require us to focus not only on a population's overall or average health, but also on health equity-whether everyone has a fair and just opportunity to be as healthy as possible, which includes whether the resources, opportunities, and conditions required for good health are distributed equitably within the population"--
