Introduction : Les antivitamines K (AVK) sont le traitement anticoagulant de référence dans la prévention du risque thrombo-embolique lié à la fibrillation atriale (FA). Ces dernières années ont vu l'émergence de nouvelles molécules, les anticoagulants oraux directs (AOD), présentés comme une alternative possible aux AVK. Objectif : Evaluer la tolérance, l'efficacité et la sécurité d'emploi des AOD dans une cohorte de patients à l'échelle locale sur une durée de un an et comparer les résultats obtenus à ceux des grandes études de référence. Matériels et méthodes : L'étude est prospective, monocentrique et observationnelle. La période d'inclusion, de septembre 2013 à septembre 2014, a concerné les patients à risque thrombo-embolique atteints de FA chez qui a été instauré un AOD à l'hôpital de Pau. Un questionnaire a été élaboré, avec des questions spécifiques destinées aux patients et à leurs médecins traitants. Les réponses à ces questions ont été obtenues au cours d'un entretien téléphonique réalisé auprès de chaque médecin et patient en capacité de répondre, entre janvier et septembre 2015. Résultats : 106 patients ont été inclus. Le suivi médian a été de 18 mois. La moyenne d'âge de la population étudiée était de 73,9 ans. La tolérance du traitement a été jugée bonne par 78,3% des patients interrogés. Aucune complication thrombo-embolique n'est survenue au cours du suivi. Des évènements hémorragiques ont été relevés chez 18,9% des patients : 5,7% d'hémorragies majeures dont 0,9% de fatale et 13,2% d'hémorragies mineures. Les résultats obtenus sont cohérents avec ceux des études princeps et post-AMM publiés à ce jour. A l'issue du suivi, 73,6% des patients poursuivaient le traitement par AOD qui leur avait été prescrit et 26,4% l'ont arrêté de manière définitive. Plus de la moitié des arrêts l'ont été pour des raisons non justifiées. Conclusion : Dans la prise en charge de la FA, le traitement anticoagulant fait appel à des molécules puissantes au risque iatrogène certain mais à l'intérêt prouvé et incontestable. Les AOD ont montré leur non infériorité voire leur supériorité sur certains critères par rapport aux AVK, à travers des études princeps à large échelle et de vastes études post-AMM et en vie réelle. Les études futures permettront de préciser la place réelle qu'occupent les AOD dans le traitement du trouble du rythme le plus fréquent de la population générale et enjeu de santé publique.
