L'observance est un problème majeur de nos jours, à tel point que l'OMS a déclaré en 2003 que "résoudre la non observance thérapeutique serait plus efficace que n'importe quel nouveau projet biomédical". Cette formulation démontre l'importance que les autorités et les professionnels de satné doivent accorder à ce sujet. En France, la mauvaise observance concerne 60% des patients attients d'une maladie chronique. Ce défaut d'observance a des conséquences se répercutant directement sur le patient avec une dégradation plus rapide de sa qualité de vie, mais aussi sur le système de santé d'Etat qui doit alors ouvrir des dépenses plus conséquentes. L'observance est aujourd'hui un sujet qui est porté sur le devant de la scène par les études réalisées principalement dans le cadre particulier d'une pathologie donnée ou chez la personne âgée. Pourtant les patients concernés par une mauvaise observance peuvent cumuler plusieurs maladies chroniques ou déficiances. Il est donc nécessaire d'élargir l'accompagnement renforcé à toutes les personnes fragiles fréquentant l'officine sans limitation d'âge ou de maladie. Le pharmacien d'officine est le professionnel de santé référent pour la médication des patients. Il est donc légitime qu'il se trouve au premier plan pour la sensibilisation des patients lors des primo-prescriptions, puis la prévention et le dépistage d'une mauvaise obervance auprès des personnes fragiles. Pour mener cet accompagnement personnalisé à bien, le pharmacien doit mobiliser l'ensemble de ses collaborateurs qui rencontre le patient au comptoir, et pour sa mise en oeuvre, il peut s'appuyer à la fois sur des dispositifs mis en place par l'Etat et sur des propositions de laboratoires ou groupements de pharmacies. Pour une efficience améliorée de la prise en charge du patient, le pharmacien doit être impliqué dans une coopération interprofessionnelle avec les autres professionnels de santé acteurs du parcours de soins du patient, voire impliquer l'entourage familia et amical du patient.
