En 1997, l'association rouennaise Soins Pour Tous a été créée sur l'initiative de quelques bénévoles infirmiers, médecins et pharmaciens. L'objectif de cette association était d'apporter une aide à une partie de la population qui n'accédait pas au système de soins. En 2006, l'association continue son action. C'est une des seules unités mobiles en France à proposer des soins aux sans domiciles fixe, dans la rue, la nuit. Cette population est peu explorée, et le travail médical associatif est peu connu. Nos résultats d'études sur huit ans, montrent qu'il n'existe pas de pathologies spécifiques de la précarité ou de la grande exclusion. Les pathologies rencontrées sont la plupart du temps évoluées du fait des obstacles à l'accès aux soins encore existant, obstacles institutionnels ou propres à l'individu. La prise en charge du patient précaire est spécifique car elle nécessite de repenser la prise en charge classique basée sur la notion d'efficacité thérapeutique et d'inventer un nouveau lien thérapeutique. Ce lien spécifique nécessite de la part du soignant une compréhension des processus de transfert et contre-transfert et la notion d'objet transitionnel. Cependant, la prise en charge des patients désocialisés par les associations pose le problème d'une prise en charge ponctuelle et elle-même précaire car soumis aux disponibilités des bénévoles, aux subventions, aux différents dons. Cette précarité subie une fois de plus par les précaires d'entre nous doit-elle se perpétuer et pérenniser ainsi un système excluant? On peut envisager la remise en place de structures basées sur le concept de dispensaire, d'accès libre, sous forme de dépendances du service publique et en lien avec l'hôpital. Ces structures existent, elles sont représentées par les PASS. Il suffit d'une volonté politique afin d'offrir aux plus précaires une structure cadrante, accueillante, permettant un accès aux soins effectifs, loins des obstacles institutionnels et individuels.

This book deals with slums as a specific question and a central focus in urban planning. It radically reverses the official version of the history of world cities as narrated during decades: slums are not at the margin of the contemporary process of urbanization; they are an integral part of it. Taking slums as its central focus and regarding them as symptomatic of the ongoing transformations of the city, the book moves to the very heart of the problem in urban planning. The book presents 16 case studies that form the basis for a theory of the slum and a concrete development manual for the slum. The interdisciplinary approach to analysing slums presented in this volume enables researchers to look at social and economic dimensions as well as at the constructive and spatial aspects of slums. Both at the scientific and the pedagogical level, it allows one to recognize the efforts of the slum’s residents, key players in the past, and present development of their neighborhoods, and to challenge public and private stakeholders on priorities decided in urban planning, and their mismatches when compared to the findings of experts and the demands of users.
