Introduction : les patients repérés comme fragiles par le médecin généraliste sont adressés et évalués à l'hôpital de jour des fragilités. Notre objectif principal était de recueillir auprès de ces sujets âgés avant leur évaluation leurs représentations de la fragilité ainsi que leurs situations de fragilités vécues. Nos objectifs secondaires étaient de recueillir les stratégies compensatrices pour lutter contre la fragilité et les attentes de leur passage à l'hôpital de jour des fragilités. Matériels et méthodes : nous avons réalisé une étude qualitative avec des entretiens individuels semi-dirigés. Résultats : 15 entretiens ont été analysés. Les représentations de la fragilité et les situations de fragilité vécues les plus abordées par les sujets interrogés concernaient le domaine physique et socio-économique. Les sujets mettaient spontanément en place des stratégies compensatrices pour lutter contre la fragilité. Les attentes des sujets interrogés concernant l'hôpital de jour n'étaient adaptées que pour deux sujets et deux sujets n'en avaient aucune. Conclusion : les représentations de la fragilité et les situations vécues des sujets adressés à l'hôpital de jour étaient diverses, hétérogènes et différentes de la définition gériatrique de la fragilité. Il existait une confusion d'une part entre fragilité et vieillissement et d'autre part entre fragilité et dépendance. Les résultats de notre étude pourraient être utilisés pour améliorer la prise en charge des futurs patients adressés à l'hôpital de jour. Enfin, il pourrait être intéressant de recueillir les représentations de la fragilité chez les médecins généralistes.

Nous avons analysé au cours de cette étude les transitions entre les statuts robuste, pré-fragile et fragile, et leurs facteurs prédictifs. Introduction : le repérage et la prise en charge de la fragilité permettent de prévenir la dépendance. Les causes de fragilité sont multiples et souvent intriquées. L'objectif de ce travail est de déterminer la part respective de chaque dimension de fragilité et d'en connaître leurs déterminants. Méthodes : nous avons mené une étude observationnelle rétrospective descriptive et analytique à partir de l'évaluation multidimensionnelle, prospective et monocentrique des patients adressés à l'hôpital de jour des fragilités du Gérontopôle de Toulouse entre septembre 2011 et novembre 2015. Résultats : parmi les 2255 patients inclus, 44.9% étaient adressés par un médecin généraliste, 38.2% par un gériatre et 17% par un médecin spécialiste ou une autre structure. Selon l'indice de Fried, 51% étaient fragiles, 40.5% pré-fragiles et 8.5% robustes. Ils présentaient en moyenne 3 dimensions de fragilité : physique pour 76.8%, sensorielle pour 71%, sociale pour 49.2%, thymique pour 30.4%, fonctionnelle pour 27%, cognitive pour 23% et nutritionnelle pour 15.7%. En analyse multivariée, l'âge (85 ans et plus comparativement à moins de 85 ans) est un facteur de risque pour la fragilité physique (OR 2.92 IC95% [1.67-5.33]), sensorielle (OR 2.19 IC 95% [1.40-3.47]) et cognitive (OR 2.64 IC95% [1.61-4.36]) mais un facteur protecteur pour la fragilité thymique (OR 0.59 IC 95% [0.38-0.91]). Le sexe féminin est un facteur de risque pour la fragilité physique (OR 2.18 IC95% [1.33-3.59]) et sociale (OR 3.73 IC 95% [2.54-5.53]). Le niveau d'étude (scolarisation comparativement à pas de scolarisation) est un facteur protecteur de fragilité sensorielle (OR 0.10 IC 95% [0.01-0.53]) avec un effet de gradient. La fragilité fonctionnelle est déterminée par la fragilité physique (OR 3.58 IC 95% [1.68-8.60]), cognitive (OR 2.21 IC 95% [1.28-2.80]) et thymique (OR 2.01 IC 95% [1.24-3.28]). Le mode de vie et la catégorie socio-professionnelle ne semblent pas déterminer le profil de fragilité. Conclusion : la connaissance de la part de chaque dimension de fragilité et de leurs déterminants, communs et spécifiques, pourrait améliorer le repérage et la prise en charge des personnes âgées fragiles par la mise en place d'interventions ciblées, fortes et prolongées.
