120 millions de boîtes de médicaments psychotropes ont été prescrits en 2004 ce qui fait de notre pays le plus gros consommateur européen de ces substances. En première ligne pour recevoir les plaintes les plus diverses, isolé dans un réseau de soin où les psychiatres se font rares, le médecin généraliste est donc le principal prescripteur des médicaments psychotropes en France. Pour aborder ce problème de santé publique existant depuis plus de dix ans, nous décrivons tout d’abord les moyens diagnostiques et thérapeutiques mis à la disposition du médecin de famille pour prendre en charge ses patients atteints de troubles dépressifs, anxieux et du sommeil; ces moyens ne sont pas toujours adaptés au quotidien. Par ailleurs, bien que volumineuse, la prescription des psychotropes rate souvent sa cible et certains patients nécessitant une prise en charge spécifique n’en bénéficient pas. Nous tentons d’expliquer les difficultés rencontrées par le médecin généraliste lors de la prescription de ces substances et nous analysons ainsi sa position particulière, représentant d’une médecine toute-puissante dans une société consommatrice de bonheur. Après avoir pointé du doigt les différents dysfonctionnements concernant tous les acteurs intervenant dans la surconsommation des médicaments psychotropes (médecin, industrie pharmaceutique, patient, système de soins), nous élaborons différentes pistes pour aider le médecin généraliste à mieux maitriser cette prescription

Le processus de decision conduisant a la prescription d'un medicament psychotrope au cours d'une consultation en medecine generale est mal connu. Une approche quantitative nous a semble peu a meme de capter l'ensemble des facteurs susceptibles de conduire a la prescription d'un medicament psychotrope. Nous avons donc choisi une approche qualitative, de type "ethnographique", dont l'objectif etait de decrire au plus prêt quelques consultations conclues par une ordonnance de psychotrope remise au malade, a l'occasion d'un stage de medecine generale chez trois praticiens. Il est apparu que cette situation etait plus rare qu'attendue, et dependait beaucoup de la personnalite du praticien et de sa clientele, ces deux facteurs n'etant pas independants. La prescription de psychotropes ne se fait pas au detriment d'une relation d'ecoute et d'aide, et celle-ci ne dispense pas, le plus souvent, de la prescription. Les antidepresseurs de type "serotoninergique" et les tranquillisants sont les medicaments les plus prescrits, pas forcement selon les donnees "de la science" car une negociation est toujours necessaire sur les representations du probleme de sante et du medicament. Le malade est parfois a l'origine de la prescription, en particulier pour les tranquilisants (consideres comme les "medicaments du malade") alors que l'initiative revient au medecin, qui doit justifier et argumenter sa prescription, pour les antidepresseurs ("medicaments du medecin"). A partir de ce travail, nous avons analyse le processus de decision conduisant a la prescription de psychotropes en distinguant les facteurs lies aux medecins et a leurs contextes d'exercice, aux malades et aux symptomes qu'ils expriment lors de la consultation, aux attentes et representations respectives des uns et des autres vis-a-vis des troubles psychologiques, et enfin au contexte social plus global de la relation medecin-malade (role de l'industrie pharmaceutique et demande sociale ambiguë vis-a-vis du soulagement du "mal-etre", par exemple).

La consommation des psychotropes en France est élevée tout comme la primo prescription des antipsychotiques par le médecin généraliste, le plus souvent hors AMM ; ceci en dépit d'effets indésirables importants aux vues des indications parfois discutables. Ainsi il semblait nécessaire de préciser les caractéristiques optimales de la primo prescription des antipsychotiques en médecine générale par la réalisation d'une revue systématique de la littérature. Trente cinq articles ont été analysés. On retrouve 2 indications notables, les troubles du comportement en cas de démence type Alzheimer et les états d'agitation. Dans les démences, la risperidone à faible dose peut-être une alternative à l'olonzapine. Le traitement devra être réévalué à 5 jours et pour une durée limitée, en moyenne 12 semaines. En cas d'agitation, la risperidone per os ou l'haldol injectable restent indiqués. Les autres indications, comme le trouble anxieux généralisé, devraient nécessiter un avis spécialisé. Un guide didactique d'aide à la prescription a été réalisé. Celui ci rappelle les indications de ces médicaments dans les troubles sus cités, conseille pour le choix du médicament le plus indiqué ainsi que les conditions d'utilisation. Il indique égélement les informations pratiques à donner au patient, ainsi que le suivi à effectuer.

How the insane asylum became a laboratory of democracy is revealed in this provocative look at the treatment of the mentally ill in nineteenth-century France. Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism.
