Introduction : La prise en charge de la santé sexuelle est une discipline difficile de la médecine générale, il s'agit pourtant d'un enjeu de santé publique majeur qui, malgré les outils déjà à disposition, évolue peu en France. Face à cette problématique un nouvel outil à été mis en place en Novembre 2017 par les pouvoirs publics : la cotation « Consultation Contraception et Prévention » (« CCP »). Il s'agit d'une consultation dédiée aux jeunes femmes de 15 à 18 ans, prise en charge à 100% en tiers payants intégral, côté à 46 euros. Elle a pour but d'inciter et d'améliorer la prise en charge de la santé sexuelle des adolescentes. Cette thèse à pour objectif de mieux comprendre l'utilisation et l'utilité d'une cotation spécifique dans la prise en charge de la santé sexuelle des adolescentes. Méthode et Matériel : Cette étude qualitative a été menée de Janvier à Décembre 2020. Un échantillonnage théorique a été effectué et le recueil des données s'est fait via l'enregistrement de focus groupes et d'un entretien semi-dirigé. Le codage a été effectué de manière indépendante par les deux chercheurs puis comparé aux données de la littérature. Conclusion : La cotation « CCP » est un nouvel outil imparfait et incomplet dans la prise en charge de la sexualité des adolescents. La sexualité est en effet un tabou sociétal qui est difficile à aborder, cependant la société évolue et la médecine doit évoluer avec la société. Inclure les sciences sociales dans la formation des médecins semble indispensable pour mieux comprendre les dynamiques en jeux et améliorer la prise en charge des adolescents mais également de l'ensemble des patients. Cette évolution de la médecine générale doit passer par une modification de la rémunération des médecins afin de prendre en compte la complexité sans cesse croissante de la médecine. Une cotation n'est pas la solution mais fait partie des outils nécessaire à l'évolution dans la prise en charge de la santé sexuelle. D'autre éléments semblent nécessaires et se développent en France : la pluridisciplinarité (via les MSP notamment), le rôle de l'Etat, des médias et des associations locales. L'enseignement dès le plus jeune âge concernant des domaines comme la sexualité mais également la formation médicale initiale et continue sont autant d'éléments qui doivent être pris en compte afin d'améliorer la prise en charge les patients dans leur ensemble mais également au niveau individuel.

Contexte : La feuille de route de la Stratégie Nationale de la Santé Sexuelle (SNSS) de 2018-2020 avait comme ambition à l'horizon de 2030 d'agir précocement auprès des jeunes pour permettre à chacun de prendre des décisions éclairées et responsables quant à leur santé sexuelle. Les professionnels de santé par leurs attitudes et connaissances ont un rôle déterminant au même titre que les parents, l'entourage ou l'école dans les choix en lien avec leur sexualité qui seront faits par les jeunes. C'est dans ce contexte que l'Assurance Maladie a mis en place la cotation CCP (Consultation Contraception et Prévention) avec une rémunération à 46€ au lieu de 25€. Ce travail avait pour objectif de recueillir l'opinion des médecins généralistes du Bas-Rhin et leur pratique depuis cette revalorisation. Méthode : Il s'agit d'une étude qualitative réalisée par des entretiens semi-directifs chez des médecins généralistes installés dans le Bas-Rhin. Les entretiens se sont déroulés entre 2020 et 2021, en face à face au sein de leur cabinet médical. Résultats : Tous les médecins interrogés étaient favorables à une revalorisation de cette consultation jugée chronophage et parfois complexe. Cependant, plusieurs freins ont été mis en évidence. La tranche d'âge de cette cotation apparait comme trop restrictive et ne semble pas adaptée à la pratique de la plupart des médecins. Une mauvaise information autour de cette cotation a consisté à accentuer l'absence de réalisation de cette cotation. De plus, le thème de la sexualité n'était pas toujours aisé à aborder par les différents participants, notamment lorsque les parents accompagnaient les patientes. Plusieurs perspectives d'améliorations ont émergé de cette étude, notamment améliorer la formation des médecins sur ces sujets, étendre cette CCP en la généralisant aux garçons et en faisant évoluer la tranche d'âge, en créant un outil d'aide à la prescription de la contraception et améliorer l'information autour de cet acte. Conclusion : Bien que les participants étaient unanimes sur la nécessité de revaloriser leur travail surtout lors de consultations chronophages comme celle-ci, aucun d'entre eux n'a réellement modifié sa pratique dans les suites de cette cotation. Plusieurs freins ont été relevés mais des pistes d'amélioration ont été proposées.

Throughout the chapters, the authors try to identify other relevant factors that influence the outcomes we discuss or that influence the impact of population growth on those outcomes.
