Objectif : Mettre en évidence les facteurs déterminants le projet d'allaitement initial chez la mère professionnellement active. Méthode : Etude prospective, en deux temps, réalisée du 22 juin 2016 au 09 janvier 2017 sur 3 établissements du réseau PACA Est - Corse - Monaco, dans le service de suites de couche. 141 patientes incluses à la maternité, 83 ont répondu à distance du post-partum. Résultats : 47 mères (33,33 %) souhaitent suivre les recommandations de la HAS, la connaissance de celles-ci favorise un désir d'allaitement de plus longue durée. A contrario, une séparation précoce, due au congé maternité trop court ou à la difficulté économique à prendre un congé parental, oriente 48,94 % des patientes à un sevrage précoce avant les six mois du nourrisson. Au vu du peu d'intérêt porté aux aménagements possibles au travail (heure d'allaitement, locaux dédiés...). Conclusion : La reprise du travail est un frein tant pour l'élaboration que pour la réalisation du projet d'allaitement. Une information et un soutien adéquats des mères sont indispensables pour changer leur représentation. Un renforcement de la formation des professionnels ainsi que l'évolution des politiques de santé sont des axes d'amélioration pour permettre aux femmes d'élaborer en toute liberté leur projet d'allaitement.

Ces investissements auraient toutefois des retornbees substantielles : 65 millions de cas de retard de croissance et 265 millions de cas danemie chez la femme auraient ete evites en 2025 comparati vement a I' annee de reference 2015. En outre, sur 10 ans, au moins 91 millions d' enfants supplementaires auraient ete traites pour emaciation severe et 105 millions de nourrissons additionnels auraient ete beneficiaires d'un allaitement matemel exclusif au cours de leurs premiers six mois de vie. L'atteintede l'ensemble de ces cibles permettrait d'eviter au moins 3,7 millions de mortalites infantiles. Chaque dollar investi dans ce paquet d'interventions genererait des rendements econorniques de4 a 35 dollars, ce qui fait du financement de la nutrition precoce l'une des interventions de developpement les plus rentables. Certaines des cibles - particulierement celles qui sont axees sur la reduction du retard de croissance chez l'enfant et de I'anemie chez la femmeparaissent ambitieuses et demanderont des efforts concertes de financement, de mise a l' echelle et d' engagement continu; toutefois, l' experience tiree de plusieurs pays indique que leur atteinte reste possible. Les investissements au cours des 1000 premiers jours decisifs de la petite enfance sont inalienables, transportables et porteurs de retombees tout au long de la vie - non seulement pour les enfants directement concernes, mais aussi pour nous tous - sous forme de societes plus robustes, qui agissent comme moteur des economies a venir.

Against the larger backdrop of community attitudes that variously sustain or restrain breast-feeding the 32-page booklet translates the most up-to-date knowledge and practical experience about lactation into precise recommendations on care for mothers before during and after pregnancy and delivery. The statement begins by listing 10 important steps to successful breast-feeding intended for application in every facility providing maternity services and care for newborn infants. Readers are told that mothers should be helped to breast-feed within a half hour of birth that newborn infants should be given no food or drink other than breast milk unless medically indicated and that rooming-in should be practised 24 hours a day. Particularly practical is a section devoted to individual care, which spells out procedures to follow at five important stages from prenatal history-taking and counseling through care during and after delivery to what to do when a mother is discharged from the health care facility.
