Le vieillissement démographique de la population française constitue un défi sanitaire et social, auquel le système de soins doit s’adapter ; en effet, de plus en plus de patients sont âgés, fragiles, polypathologiques et dépendants. Leur état de santé engendre des problématiques de prise en charge souvent complexes, aux dimensions médicales, sociales, psycho-comportementales et fonctionnelles. Une prise en charge gériatrique spécifique et globale est recommandée pour optimiser le devenir de ces patients âgés ; elle s’exerce idéalement au sein d’une filière de soins gériatriques hospitalière. Le Centre Hospitalier de Seclin possède une telle filière depuis l’ouverture en Septembre 2006 du service de Court Séjour Gériatrique. Notre étude a permis d’analyser les caractéristiques sociales, environnementales, fonctionnelles et médicales des patients hospitalisés dans ce service de Court Séjour Gériatrique, durant ses six premiers mois d’activité ; elle a aussi permis de mettre en évidence les spécificités des missions et du fonctionnement de ce service, ainsi que les améliorations à y apporter. Nos résultats définissent le profil type du patient pris en charge en Court Séjour Gériatrique : il s’agit d’une femme, âgée de 83 ans, vivant seule à domicile avec des aides ménagères, dans le secteur de Seclin. Elle est hospitalisée par son médecin traitant, via les urgences, pour un syndrome gériatrique aigu. A son admission en Court Séjour Gériatrique, cette personne âgée présente une comorbidité importante et une polymédication ; elle est dépendante, dénutrie et présente des troubles cognitifs. Elle bénéficie durant son hospitalisation d’une prise en charge globale et pluridisciplinaire ainsi que d’examens paracliniques, qui permettent d’établir un diagnostic gériatrique fréquemment associé à une pathologie infectieuse. Après des soins médicaux, une rééducation par un kinésithérapeute et un bilan social, elle retourne dans son milieu de vie initial au bout de dix jours d’hospitalisation en Court Séjour Gériatrique. Si son état nécessite une convalescence, la patiente est accueillie dans le service de Soins de Suite et de Réadaptation du Centre Hospitalier de Seclin. Le service de Court Séjour Gériatrique prend donc en charge des patients âgés répondant aux critères de fragilité. Il parvient à remplir sa mission, assurant une prise en charge globale du patient et limitant les risques de perte d’autonomie fonctionnelle liés à une hospitalisation. Ce résultat est obtenu grâce à un travail concerté avec ses partenaires de la filière gériatrique, et la possibilité de recours à des compétences multiples et pluridisciplinaires. Ainsi, le Court Séjour Gériatrique apparaît comme un progrès nécessaire dans la prise en charge des sujets âgés, aux retombées bénéfiques avant tout pour ces patients, mais aussi pour le système de soins hospitalier

L’effectif de la population âgée continue de croitre. La politique d’offre de soins doit être adaptée à cette évolution démographique. Dans le service de médecine gériatrique du Dr Sebbane (hôpital René Muret-Bigottini, Sevran, Seine-Saint-Denis), il existe une unité de dix lits de court séjour gériatrique travaillant en étroite collaboration avec les médecins généralistes de secteur et les services des urgences des hôpitaux d’Avicenne (Bobigny) et de Jean Verdier (Bondy). Notre étude, rétrospective sur 6 mois, s’est attachée à décrire et à analyser les profils à l’entrée et le devenir des malades gériatriques hospitalisés dans cette unité. Les résultats montrent que 80% des malades sont polypathologiques et polymédiqués. 33% sont pris en charge pour deux diagnostics au moins. 75.2% ont des fonctions supérieures altérées et une motricité réduite. Plus de 50% sont incontinents urinaire et ont une hypoalbuminémie. Un patient sur sept présente une ou plusieurs escarres et une carences en folates. A 1 mois, un patient sur 2 est pris en charge en SSR, 1 sur 6 est rentré au domicile, plus 1 sur 7 est décédé et 1 sur 12 est en fin de vie. A 6 mois, un patient sur 6 est encore pris en charge en SSR, 1 sur 10 en USLD et en EHPAD. 1 malade sur 5 est rentré à domicile et 1 sur 3 est décédé. Les taux moyen d’albumine des patients diffèrent significativement (p= 0.005) selon que les patients sont, ou décédés (30,9 ± 4,7 g/l),ou en fin de vie (35,1 ± 6,8 gIl), ou vivants (35,9 ±4,7 g/l.) à 1 mois de leur admission. Ces résultats confirment le profil fragile des malades hospitalisés en unités de court séjour gériatrique. Les unités de court séjour gériatrique, au sein de la filière de soins, comportent des forces et des fragilités. Elles doivent cependant pérenniser une triple vocation de soins, d’enseignement et de recherche.

Phronesis is the Aristotelian notion of practical wisdom. In this collected series, phronesis is explored as an alternate way of considering professional knowledge. In the present context dominated by technical rationalities and instrumentalist approaches, a re-examination of the concept of phronesis offers a fundamental re-visioning of the educational aims in professional schools and continuing professional education programs.
