L'hémorragie du post-partum (HPP) concerne 5% des accouchements et demeure la 1ère cause de décès maternel en France. Des recommandations pour la pratiques clinique (RPC) éditées en 2004 codifient sa prise en charge et la place des techniques invasives d'hémostase dont l'embolisation. L'objectif principal de notre étude était d'évaluer l'évolution de la prise en charge anesthésique et la réanimation des parturientes embolisées au cours du temps. Nos objectifs secondaires recherchaient une variation du taux d'embolisation et de la gravité des patientes. Etude rétrospective de 2001 à 2011 des cas d'embolisation en contexte d'HPP réalisées en urgence au CHU de Clermont-Ferrand. 237 cas d'embolisations chez 232 parturientes avec un taux de succès de 95%. La moitié des accouchements avaient eu lieu par césarienne. 80% des cas avaient reçu des analogues des prostaglandines, 49% avaient été transfusées en CCG, 35% en PFC et 10% en plaquettes. A compter de 2008 : 26 cas d'administration d'acide tranxénamique et 19 cas d'administration de fibrinogène; 3 cas d'utilisation de facteur VII activé. 4 hystérectomies d'hémostase. Aucun cas de décès maternel. On note une diminution du taux d'embolisation à compter de 2008, pour les patientes du CHU et de l'extérieur, associée à une augmentation statistiquement significative de la pression artérielle moyenne (PAM) en début et fin de procédure d'embolisation dans le temps. Pas de corrélation établie entre l'augmentation de la PAM et les traitements médicaux suivants (analogues des prostaglandines, acide tranéxamique, fibrinogène, CCG et PFC). Les agents pro-hémostatiques occupent à ce jour une place essentielle dans la gestion de l'HPP. Une politique transfusionnelle plus précoce et agressive, avec un ratio CCG/PFC = 1 et l'administration précoce de plaquettes, a égalemnt été développée. Le relai local de ces messages et la formation des équipes a pu avoir lieu grâce à la politique du Réseau de Santé Périnatale d'Auvergne à partir de 2007, permettant ainsi une diminution du taux d'embolisation à compter de 2008 avec une amélioration de l'état hémodynamique des patientes arrivant sur le plateau de radiologie interventionnelle.

Introduction : Les hémorragies du post-partum constituent la première cause de mortalité maternelle en France et représentent une priorité nationale de santé maternelle. Les hémorragies de la délivrance par atonie utérine en sont les principales causes. Une politique d'amélioration de la prise en charge de cette pathologie a été mise en œuvre depuis 2006 au sein du réseau santé périnatal d'Auvergne, par la mise en place et un suivi des analyses des pratiques professionnelles, dans le but de diminuer la morbi-mortalité maternelle. Les sages- femmes sont des acteurs de première ligne dans la prévention, le dépistage et la prise en charge des hémorragies immédiates du post-partum. Ce travail a été réalisé dans le but d'évaluer l'évolution de ce travail de prévention et de traitement de l'hémorragie du post-partum au sein du Centre Hospitalo-Universitaire d'Estaing à Clermont-Ferrand. Matériels et méthodes : Nous avons travaillé sur l'ensemble des accouchements, au-delà de 22SA, par voie basse et par césarienne, compliqués d'une hémorragie de la délivrance entre le 01/01/2012 et le 31/12/2019 au CHU de Clermont-Ferrand. Résultats : Les chiffres montrent que l'incidence des hémorragies du post-partum augmentent au CHU Estaing entre 2012 et 2019 pour les accouchements voie basse et diminue pour les accouchements par césarienne. L'incidence des hémorragies sévères du post-partum reste stationnaire au fil des années. De plus l'incidence des traitements de seconde ligne diminue (embolisation, transfusion, chirurgie). Conclusion : Même si l'incidence de l'HPP semble être en légère augmentation sur les années 2018 et 2019, l'incidence des thérapeutiques de seconde ligne, quant à elle diminue. Cela démontre une qualité du diagnostic précoce et de la prise en charge de l'HPP au CHU Estaing.

Despite increased academic interest in both children's rights and socio-economic rights over the last two decades, children's social and economic rights remain a comparatively neglected area.
