Introduction : Les migrants à Nantes sont d'âge, de sexe et de pays d'origine différents. Le médecin généraliste rencontre parfois des difficultés inhérentes aux migrants et cela semble encore plus compliqué lorsqu'il s'agit de la symptomatologie dépressive. Méthode : Le choix s'est porté sur une enquête qualitative par entretiens semi-directifs auprès de médecins généralistes nantais. Résultats : Consultés par des patients d'autres cultures, les médecins généralistes rencontrent des difficultés de communication pour comprendre la demande qui leur est adressée, transmettre la réponse qu'ils proposent et en évaluer la pertinence. En ce qui concerne la dépression, les références culturelles amènent une difficulté supplémentaire puisque les représentations et l'expression clinique de la maladie sont différentes. Les plaintes somatiques sont souvent au premier plan. Les médecins interrogés ne savent pas vers qui s'orienter. Interprètes professionnels, structures, spécialistes et formations seraient d'une grande aide. Discussion : La barrière de la langue et les représentations culturelles sont des difficultés inhérentes aux patients migrants. Chaque histoire est singulière. C'est cette singularité qu'il faut prendre en compte, en se posant la question avec le patient de savoir pourquoi il souffre aujourd'hui. La souffrance psychique des patients migrants requiert une prise en charge spécifique dans la manière de recevoir et de concevoir la parole et l'expression du patient. La connaissance des systèmes de références du patient est un atout précieux du médecin dans l'investigation diagnostique et dans le choix d'une thérapeutique adaptée. Le recours à un interprète, autre atout important, permet d'instaurer un climat de confiance, de traduire les mots mais aussi d'interpréter des concepts différents.

La psychiatrie transculturelle est l'utilisation des représentations culturelles dans la prise en charge de la dépression. Cependant les recommandations de la Haute Autorité de Santé (HAS) ne l'abordent pas. L'objectif est de décrire la prise en charge ambulatoire actuelle des patients migrants maghrébins dépressifs en médecine générale. La question est : les généralistes appliquent-ils pareillement, chez les patients maghrébins et autochtones, les recommandations de la HAS? Qu'en est-il de la psychiatrie transculturelle? Les hypothèses sont: Les recommandations sont appliquées différemment. La psychiatrie transculturelle influence la prise en charge. On fait une étude observationnelle par questionnaire, chez des généralistes traitant des dépressifs maghrébins et autochtones. Malgré la petite taille de l'échantillon, on voit une différence avec une meilleure utilisation de la psychiatrie transculturelle chez les migrants. Ce résultat pourrait inciter à étudier ses outils.

Based on a recently completed project of cultural consultation in Montreal, Cultural Consultation presents a model of multicultural and applicable health care. The influx of African migrants into Europe in recent years has raised important issues about changing labor economies, new technologies of border control, and the effects of armed conflict. But attention to such broad questions often obscures a fundamental fact of migration: its effects on ordinary life. Affective Circuits brings together essays by an international group of well-known anthropologists to place the migrant family front and center. Moving between Africa and Europe, the book explores the many ways migrants sustain and rework family ties and intimate relationships at home and abroad. It demonstrates how their quotidian efforts—on such a mass scale—contribute to a broader process of social regeneration. In the closing decade of the last century, we saw warnings that infectious diseases will require much more attention from patients and physicians in the 21 st century. Recently d- covered diseases such as AIDS pose a major threat to the population at large, and to that threat has been added the re-emergence of established pathogens, microbes that were re- ily treatable in the past. Since infectious diseases already play a major role in the burden of illness and mortality, health care providers and planners are worried. A large proportion of the problem is man-made, arising mainly from the unnecessary overuse of antimicrobials in hospital and community settings and from the agricultural misuse of the agents in animal feed. A consequence has been a dramatic increase in resi- ant strains of bacteria that were considered conquerable several decades ago. Community infections caused by multi-resistant pneumococci serve as an example. These organisms were readily treated with penicillin, but now the spread of penicillin-resistant Streptococcus pneumoniae from continent to continent is becoming a worldwide problem. This is a major concern because pneumococcal infections are common in the community, being the le- ing cause of pneumonia, sinusitis, and meningitis. Resistant bacteria in hospitals are also becoming more prevalent. We have become accustomed to hearing about methicill- resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE), but now we have to be concerned about multidrug-resistant coliform bacteria and pseudomonads.
