Introduction : La douleur en lien avec le cancer est un symptôme d'inconfort majeur. Son traitement peut nécessiter le recours aux antalgiques opioïdes forts. La majoration de leur prescription ces dernières années est associée à une augmentation considérable de leur usage inapproprié. Compte-tenu des risques encourus par les patients, dépister cet usage inapproprié est un enjeu de Santé Publique. Or sa prévalence n'est pas connue et il n'existe pas d'échelle de dépistage de ce risque validée en population cancéreuse. Objectif : L'objectif principal était de déterminer la prévalence de l'usage inapproprié en antalgiques opioïdes chez les patients présentant des douleurs chroniques en lien avec le cancer, à partir d'une analyse de la littérature. Les objectifs secondaires étaient d'identifier les caractéristiques de ces patients et développer une échelle de dépistage de cet usage inapproprié. Méthode : Il s'agit d'une analyse de littérature d'articles publiés entre janvier 2000 et décembre 2020, issus des bases de données médicales PubMed, Cochrane et Embase. Nos critères d'inclusion étaient les études traitant de l'usage inapproprié des antalgiques opioïdes forts dans le traitement de la douleur chronique en lien avec le cancer. Résultats : Sur les 1195 articles initialement sélectionnés, 15 ont été inclus dans l'analyse statistique. Nous retrouvons une prévalence de 8% (1%-20%) pour l'usage inapproprié des antalgiques opioïdes forts constaté chez les patients atteints de douleurs chroniques en lien avec le cancer. L'hétérogénéité entre les études est forte avec un I2 de 97.8%. Conclusion : Notre étude prouve un manque de données concernant l'usage inapproprié des antalgiques opioïdes fort parmi les patients traités pour douleurs chroniques en lien avec le cancer. De nouvelles études doivent être menées pour créer et valider une échelle adaptée au dépistage de cet usage inapproprié et permettre ainsi une estimation de sa prévalence qui reflète la réalité.
