Many clinicians have reservation about the different types of treatments for pressure ulcers (PUs). The medical field becomes so much more complex when an elderly patient comes in with multiple diseases and co-morbidities, and has developed a pressure ulcer (PU). This will change the whole course of treatment, which can cause complications that may prolong healing times, and can eventually lead to death. It is critical to a geriatric patient to practice in depth knowledge of appropriate and aggressive PU healing techniques for the treatment to become effective ( Yasturb, 2005). As the years have passed clinicians have used many different methods for treatments. The use of dressings, adequate nutrition, and mobility has led researchers to their best outcomes in PU healing. The elderly population tends to be malnourished as there muscle mass weakens, and there appetite decreases, therefore nutrition is a high priority especially if a pressure ulcer is present to increase healing. The rate of PU healing appears to accelerate when a nutrition formula enriched with protein, arginin, zinc, and vitamin C is administered (Cereda, Gini, Pedrolli, Vanotti, 2009). Immobility has become a common complication among the elderly population, resulting is substantial pain and suffering. Frequent manual repositioning has also become an established part of PU prevention that healthcare facilities have adopted into their standard of care. The reasoning behind this is the thought of frequent repositioning will decrease pressure off of bony prominences, which many theorists have thought will decrease the risk of developing a PU (Rich, Margolis, Shardell, Hawkes, Miller, Amr, and Baumgarten, 2011).

Abstract: Older adults are particularly vulnerable to the development of pressure ulcers (PUs) as a result of skin changes and reduced mobility (Knox, Anderson & Anderson, 1994; Russell et al., 2003). This is associated with diminished quality of life, longer hospitalisations and increased morbidity and mortality (Margolis et al., 2002).In view of the high incidence of PUs (grade 1-4) occurring in elderly patients in acute care, a systematic review of related studies was conducted in August 2011. The optimal frequency and methods of repositioning are described in the literature. With well-designed implementation and evaluation plans, the proposed repositioning guidelines are likely to reduce the incidence of PUs (grade 1-4), while in turn lessening the healthcare burden and preserving patients' quality of life. Pilot testing, a trial run to test the feasibility of the innovation, will be carried out in the proposed medical ward, and training provided to all staff before the intervention. To proceed with the change, the intervention must be cost-effective and beneficial to all stakeholders. Outcome evaluation determines the number of goals achieved by the innovation and to what degree, and is very important (Melnyk & Fineout-Overholt, 2005).The PU incidence (grade 1-4) is expected to be different after the implementation of the innovation. Background Pressure ulcers, also known as “decubitus ulcers”, “pressure injuries” or more commonly “bed sores” occur when a patient is sitting or lying on a pressure point for an extended period of time, resulting in skin breakdown (Cutugno et al. 2015). Pressure ulcers can occur in any age group but most commonly occur in the geriatric population, especially those who are bed- or wheel-chair bound (Venes 2015). Being a preventable issue, pressure ulcers are a huge problem and cost our healthcare system billions of dollars annually (Cutugno et al. 2015). Pressure Ulcers also cause patients to experience embarrassment, anxiety, depression, and an increased risk of death by five times in the geriatric population (Aljezawi, Al Qadire, and Tubaishat 2014 p.6). Certified Nurses’ Assistants (CNAs) and Personal Care Assistants (PCAs) are the primary caregivers in long term care facilities to geriatric patients. CNAs and PCAs have the responsibility to assist with preventing pressure ulcers as well as documenting and communicating with nurses about the new formation of pressure ulcers.
