INTRODUCTION : La gestion des situations de soins palliatifs est fréquemment rencontrée en médecine générale, et ponctuée d'épisodes aigus motivant souvent une hospitalisation en urgence. L'objectif est de déterminer si des prescriptions anticipées personnalisées (PAP), proposées par le réseau de soins palliatifs départemental des Bouches-du-Rhône (PSP13), permettent le maintient au domicile. METHODE : Étude rétrospective des fiches d'astreinte et des dossiers médicaux des équipes mobiles de soins palliatifs du réseau, comparant les premiers trimestres 2011 et 2013, ne notant les cas d’hospitalisation, le lieu de décès, le type de complications survenus, l'utilisation de PAP, et les facteurs limitant le maintien à domicile. RESULTATS : Ont été inclus 52 patients au premier trimestre 2011 et 85 en 2013 dont 52 ont reçu des PAP. Baisse de la prévalence de la douleur (p=0,021) avec recours aux morphiniques injectables (p=0,005). La seconde complication retrouvée concerne les troubles respiratoires, dont la prévalence est à la hausse (p=0,391) malgré le recours à l'hypnovel à domicile (p=0,033). Le chois du patient de mourir au domicile est respecté (p=0,004). Les difficultés liées à l'entourage familial au cours de deux périodes (p=0,048 en 2011 et p=0,004 en 2013) et la présence d'enfant (p=0,037) sont des facteurs limitant le maintien à domicile. CONCLUSION : L'utilisation de PAP permet de soulager rapidement un patient en soins palliatifs à domicile. Le médecin généraliste est ainsi épaulé dans sa prise en charge par le réseau PSP13. Un soutien au domicile, social, psychologique ou financier doit être entrepris pour les familles rencontrant des situations de fragilité.

Introduction : Les réseaux de soins palliatifs ont pour missions principales, la prise en charge globale des patients et la coordination des professionnels entre la ville et l'hôpital. Le réseau Arc-en-Ciel fait partie de ces réseaux; en élaborant un plan personnalisé de santé pour chaque patient inclus. L'objectif de ce travail est d'évaluer l'apport du plan personnalisé de santé (PPS) dans la prise en charge à domicile des patients en soins palliatifs, et de trouver les freins à son déploiement et les outils nécessaires pour son amélioration. Méthode : Une étude descriptive a été réalisée par le biais d'un questionnaire interrogeant les professionnels de santé travaillants en collaboration avec le réseau Arc En Ciel, avec un recueil de données effectué entre février et octobre 2015. 142 professionnels ont été interrogés sur le contenu du PPS, le délai de réception, l'adéquation de cet outil à leurs besoins, leurs opinions sur son utilité et les moyens pour l'améliorer. Résultats : 72 professionnels ont répondu au questionnaire, soit 50,7%. L'impact du PPS dans la prise en charge à domicile est jugé positif, ainsi que dans l'amélioration de la coordination entre professionnels dans 68% des cas. Les freins retrouvés à son déploiement sont, entre autres : le retard de réception, le manque de fiches des effets secondaires des traitements, le manque de coordonnées des professionnels impliqués dans la prise en charge. La démarche palliative est jugée satisfaisante mais l'association des prescriptions et des directives anticipées ainsi que les coordonnées de la personne de confiance sont jugées nécessaire. Conclusion: Le PPS semble être un outil améliorant la prise en charge à domicile des patients en soins palliatifs, ainsi que la coordination entre professionnels. Tenir compte de tous ces freins en améliorant sa durée d'envoi, son contenu en y ajoutant des fiches spécifiques de thérapeutiques avec leurs effets secondaires, leurs prises en charge et les prescriptions anticipées, associer les coordonnées des intervenants ainsi que les coordonnées de la personne de confiance peut améliorer la prise en charge à domicile des patients.

Selon l' Observatoire National de la Fin de Vie, 81% des français souhaitent "passer leurs derniers instants chez eux". En outre, 30 jours avant le décès, 70% des patients vivent à leur domicile. Les soins palliatifs sont pluridisciplinaires. L' accompagnement médical et pharmaceutique du patient est essentiel à la bonne comphréhension de sa pathologie et de ses traitements. Dans le contexte des soins palliatifs à domicile, la coopération médecins-pharmaciens est-elle habituelle ? Quelles représentations ont-ils de leur coopération ? L' objectif principal de cette étude est donc d' explorer les représentations de chacun des protagonistes de cette coopération. La première partie présente les résultats d' une recherche bibliographique sur le rôle du pharmacien en soins palliatifs. La partie suivante présente la méthodologie de notre étude quantitative réalisée auprès des médecins généralistes et des pharmaciens d' officine sur une région française via un questionnaire. La troisième partie présente les résultats sous forme d' analyse thématique des verbatims et d' analyse statistique. Selon les participants, la coopération apparaît nécessaire en soins palliatifs et notamment pour : le matériel médical, les morphiniques et les ruptures de médicaments. Pour 84,7% des participants, les pharmaciens ont un rôle dans les soins palliatifs à domicile et notamment pour l' anticipation des prescriptions, la livraison à domicile, réexpliquer les thérapeutiques et les prescriptions anticipées. Perspective : la diffusion de ses résultats permettrait aux pharmaciens de se sentir plus légitimes dans la coopération avec les médecins généralistes en soins palliatifs.
