Introduction : l'Inspection Général des Affaires Sociales a publié en janvier 2015 un rapport sur les pratiques d'utilisation de la nutrition parentérale (NP) et rappelle les recommandations de bon usage. Puis l'instruction DGOS/PF2/DGS/PP2 n°2015-85 du 20 mars 2015 a renforcé cette démarche. C'est dans ce contexte qu'un état des lieux sur la pertinence des prescriptions de NP dans une population pédiatrique a été entrepris. Matériels et Méthodes : il s'agissait d'une étude observationnelle prospective descriptive à l'hôpital des enfants du CHU de Toulouse. La population étudiée comprenait des nourrissons et enfants de plus de 1 mois avec une prescription de NP. Les variables d'intérêt ont été recueillies par l'intermédiaire d'une grille de recueil élaborée par un groupe de travail pluridisciplinaire. Elle comportait six parties : i) données sociodémographiques, ii) motif(s) d'hospitalisation, iii) données concernant la prescription de NP, iv) modalités d'administration, v) médicaments co-administrés et v) alimentations associées. Le critère de jugement principal était défini par la conformité des indications de la NP au référentiel de l'European society of paediatric gastroenterology, hepatology and nutrition. Résultats : un total de 75 patients réparti dans neuf unités pédiatriques a été inclus. La pertinence de l'indication était conforme aux recommandations pour 84% des enfants. La pertinence en fonction de l'âge était significativement différente (p = 0,007). Les apports énergétiques étaient conformes dans 33% des cas. Les vitamines hydrosolubles et liposolubles étaient prescrites respectivement dans 83% et 79% des cas toutes spécialités confondues. Un suivi des marqueurs de nutrition était retrouvé dans 47% des cas pour le poids et 45% pour l'IMC. Conclusion : une prise en charge nutritionnelle adaptée est indispensable, la nutrition entérale reste la première intention avec un coût et un risque infectieux moindre. Un référentiel de prescription va être élaboré de manière pluridisciplinaire afin d'améliorer les pratiques.
