Objectifs. Dans un contexte où de plus en plus d’étudiants font reconnaître leur diplôme en France depuis l’entrée en vigueur de la directive 2005/36/CE en 2005, l’objectif de cette étude était de décrire le parcours des internes de médecine générale qui ont fait la majorité de leur deuxième cycle à l’étranger, les difficultés et les aides qu’ils ont trouvées en France et leur projet à l’issue de leur troisième cycle. Méthode. Une enquête descriptive par questionnaire internet a été réalisée auprès des 993 internes ayant réalisé leur troisième cycle de médecine générale en France depuis l’année universitaire 2012-2013. Le recueil a duré 2 mois. Une unique relance a été réalisée. Résultats. La majorité des 298 répondants avaient réalisé leur deuxième cycle en Roumanie (51.7%), en Italie (19.1 %), en Pologne (6.0%) ou en Espagne (5.4%). Leurs motivations étaient plus particulièrement l’échec au concours de première année (45.0%) et le pays d’origine (41.9%). Ils exercent ou ont pour projet d’exercer essentiellement en France (89.7% et 74.1% respectivement). La quasi-totalité des répondants (97.7% ; IC95% [96.0–99.4]) ont déclaré avoir éprouvé au moins une difficulté pendant leur troisième cycle, plus particulièrement d’ordre administratif (51.3%) et de connaissances pratiques (50.7%). Cette proportion était similaire chez les 141 répondants nés en France (95.7%; IC95% [92.4–99.0]). La grande majorité des répondants (83.6% ; IC95% [79.4–87.8]) auraient souhaité trouver au moins un type d’aide, que seule la moitié (56.7% ; IC95% [51.1–62.3]) se sont vus proposer. Conclusions. Les aides proposées apparaissent très en deçà des besoins exprimés. Ces résultats suggèrent que l’amélioration de la situation passe par le développement d’aides dédiées sur le plan administratif et sur le plan des connaissances pratiques.

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others.
