Since the 2010 Deepwater Horizon blowout and oil spill, efforts to improve safety in the offshore oil industry have resulted in the adoption of new technological controls, increased promotion of safety culture, and the adoption of new data collection systems to improve both safety and performance. As an essential element of a positive safety culture, operators and regulators are increasingly integrating strategies that empower workers to participate in process safety decisions that reduce hazards and improve safety. While the human factors of personal safety have been widely studied and widely adopted in many high-risk industries, process safety â€" the application of engineering, design, and operative practices to address major hazard concerns â€" is less well understood from a human factors perspective, particularly in the offshore oil industry. The National Academies of Sciences, Engineering, and Medicine organized a workshop in January 2018 to explore best practices and lessons learned from other high-risk, high-reliability industries for the benefit of the research community and of citizens, industry practitioners, decision makers, and officials addressing safety in the offshore oil industry. This publication summarizes the presentations and discussions from the workshop.

The blowout of the Macondo well on April 20, 2010, led to enormous consequences for the individuals involved in the drilling operations, and for their families. Eleven workers on the Deepwater Horizon drilling rig lost their lives and 16 others were seriously injured. There were also enormous consequences for the companies involved in the drilling operations, to the Gulf of Mexico environment, and to the economy of the region and beyond. The flow continued for nearly 3 months before the well could be completely killed, during which time, nearly 5 million barrels of oil spilled into the gulf. Macondo Well-Deepwater Horizon Blowout examines the causes of the blowout and provides a series of recommendations, for both the oil and gas industry and government regulators, intended to reduce the likelihood and impact of any future losses of well control during offshore drilling. According to this report, companies involved in offshore drilling should take a "system safety" approach to anticipating and managing possible dangers at every level of operation-from ensuring the integrity of wells to designing blowout preventers that function under all foreseeable conditions-in order to reduce the risk of another accident as catastrophic as the Deepwater Horizon explosion and oil spill. In some coastal regions of the United States, such as western Louisiana, offshore oil development has long been welcomed. In others, such as northern California, it has been vehemently opposed. This book explores the reasons behind this paradox, looking at the people, the regions, and the issues in sociological and historical contexts. What has been in very short supply on this issue, as in a growing number of other cases of technological gridlock, is balanced analysis. That is what this book provides. The authors' case studies, derived from interviews with Louisiana and California residents and from environmental impact statements, demonstrate that easy answers are not the most valid ones. The region that should be considered unusual, they find, is coastal Louisiana, where historical, social, and environmental factors combine to favor the offshore oil industry. But this combination of factors, they argue, is unlikely to be found in other coastal regions of the U.S. in the near future.

In light of the importance of oil and gas in California, perhaps the discovery of gold there should be viewed as just a flash in the pan. By 1938, the cumulative value of all the gold found in the state stood at something more than two billion dollars, while the cumulative value of the oil and gas produced was more than double that sum--well over five billion dollars. The story of California oil deserves to be told, and pictures tell it best. The more than three hundred photographs in this book vividly portray the development of California's rich and colorful petroleum industry from the early exploration of the mid-nineteenth century through the boom years of the first four decades of the twentieth. Although Indians and Spanish explorers had known of and used local oil seepages for centuries and the search for commercial production had begun on several fronts in the 1850s, the actual birth date of California's oil industry may be set as 1865, with the first commercial sale of oil refined in the state (by the Stanford brothers) from a well drilled in the state (on the Matthole River in Humboldt County). The fascinating text and the impressive array of photographs here assembled reveal the variety and vigor of the development that ensued: from the "world's smallest producing lease," on Signal Hill, to the derricks sharing Huntington Beach with the bathers, to the millions of mice infesting the Taft oil field in 1926-27; from the mounted patrols keeping livestock out of the Coalinga fields to the blinking light on a fence warning motorists of a well in the middle of a Los Angeles street.
