Background and aims: Human immunodeficiency virus (HIV) infection can cause neuropsychiatric disorders such as cognitive impairment, behavioral difficulties or psychiatric symptoms u2013 for instance, mania and psychosis u2013 that are related to a wide variety of factors, including the direct effects of the virus, other cerebral infections caused by immune susceptibility, the pre-existing psychiatric conditions and personality vulnerabilities. Most of the neuropsychiatric symptoms are associated with late stage infection of HIV infection. A review of the neuropsychiatric symptoms associated with HIV infection is presented, calling attention to the inaugural presentation of HIV infection as a neuropsychiatric reference to the ER.Objectives: The present work aims to call attention to the neuropsychiatric inaugural presentation of HIV late stage infection, in the absence of any prior medical or psychiatric morbidity, reporting the admission of a patient to the ER to be evaluated by the psychiatric team.Materials and methods: Presentation of a case study, supported by a non-systematic review of the literature with the keywords u201cneuropsychiatric symptomsu201d and u201cHIV infectionu201d.Results: A 32-year-old man entered the psychiatric ER as a u201cnot identified maleu201d that was found wandering alone, disoriented, suspicious, with soliloquies but unintelligible speech. Further evaluation identified multiple ring-enhancing brain lesions associated with edema in the CT scan and HIV serology was positive. The patient was admitted to the Infectious Diseases Ward with probable cerebral toxoplasmosis.Conclusions: Considering the pleiomorphic presentation of multiple organic diseases, such causes should always be excluded in patients presenting to the ER with psychiatric symptoms.Conflicts of interest: None to declare.
