Globally, women of reproductive age face two overlapping issues that have a significant impact on their health and well-being: unintended pregnancy and sexually transmitted infections (STIs), including HIV. A growing body of research indicates that the majority of women across geographies, ages, racial and ethnic backgrounds would prefer a multipurpose prevention technology (MPT) that combines protection against pregnancy and HIV/STIs versus individual products for contraception and disease prevention. Currently, male and female condoms are the only available MPTs. A wider range of MPT options will help women select methods that they are less apt to discontinue, as well as increase uptake by first-time users. A number of MPT candidates – intravaginal rings (IVRs), oral tablets, vaginal fast dissolving inserts (FDIs), implants, injectables and microarray patches (MAPs) are in various stages of development. The goal of this Research Topic is to invigorate continued development on MPTs by providing the most up-to-date research on specific products in development, identifying research gaps related to the field overall, and prepare for introduction into programs and healthcare systems. Our broad approach will cover formulation science, clinical trials, sociobehavioral research, and implementation science to present the latest thinking in all areas of MPT development from “bench to bedside.”

One in five people in the United States had a sexually transmitted infection (STI) on any given day in 2018, totaling nearly 68 million estimated infections. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.

Global statistics on unplanned pregnancies, abortions and STIs show that unprotected sex is still widely practised. More needs to be done to provide women and men with a wider choice of convenient protective options. To address this need, international efforts are focusing on developing multipurpose prevention technologies (MPTs) that address two or more indications simultaneously. These technologies would have significant advantages over single-indication products. They include inter alia novel barrier devices, drugs administered either as oral tablets or vaginal/rectal gels, drugs used in combination with medical devices, and genetically engineered organisms which secrete antimicrobial substances. As an example of progress in the MPT field, this paper describes an on-demand contraceptive/antimicrobial vaginal gel, Amphora (previously known as Acidform), now in an advanced stage of development. Clinical trials are currently being planned to find out whether this product's promising antimicrobial profile translates into protective and preventive choices.

Unwanted pregnancies and sexually transmitted infections (STIs) are major health concerns of women worldwide. These concerns have prompted efforts to develop Multipurpose Prevention Technologies (MPTs), which simultaneously provide contraception and prevent STIs, including HIV. LL-37, the only human cathelicidin and an effective spermicide on human sperm, has broad antimicrobial activity including in vitro activity against HIV. 17BIPHE2 is a truncated LL-37 peptide, engineered to contain 5 unnatural residues, thus limiting its protease degradation within vaginal fluid. Hence, this antimicrobial peptide (AMP) represents a promising MPT agent. It was therefore hypothesized that these peptides would be inhibitors of HIV infection in cell lines, PBMC, and CD4+ T cells. In the chronically infected ACH-2 cell line, there was significant reduction in p24 production when cells were treated with 17BIPHE2, but not LL-37. When 17BIPHE2 was pre-incubated with HIV prior to infection and present during infection, viral replication decreased in the TZM-bl reporter cell line, but this result was not recapitulated in the primary activated cells, PBMCs nor isolated CD4+ T cells. Conversely, pre-incubation of 17BIPHE2 with target cells prior to infection significantly inhibited HIV infection in a dose-dependent manner. Therefore, 17BIPHE2 may act on the cell or on the virus/cell interaction rather than on the virus itself to inhibit HIV infection.
