Introduction : alors que le cancer colorectal touche près de 43 000 nouveaux patients par an en France et constitue la 2ème cause de décès par cancer, la stratégie nationale de dépistage peine à atteindre les taux de participation qu'elle s'est fixé. Au-delà de cette problématique de faible participation, une inadéquation du dépistage aux recommandations de la HAS semble également exister. Objectif : estimer l'adéquation des modalités de dépistage du cancer colorectal d'un échantillon de patients tarnais et aveyronnais aux recommandations de la HAS en fonction de leur niveau de risque vis-à-vis de cette maladie. Méthode : étude quantitative descriptive réalisée par le biais de questionnaires complétés par 26 médecins généralistes auprès de 549 patients âgés de 50 à 74 ans résidant dans le Tarn et l'Aveyron. Résultats : le taux de recrutement des médecins contactés pour participer à l'étude était de 6,4% et le taux de participation des patients éligibles de 99,6%. Le dépistage du cancer colorectal de 67,2% des patients de l'échantillon était en adéquation avec les recommandations de la HAS. Le type d'examen réalisé était en adéquation avec les recommandations pour 88,4% des patients à haut risque et 96% des patients à risque moyen. Le délai du dernier dépistage correspondait aux recommandations pour 72,8% des patients à haut risque et 71,5% des patients à risque moyen. Le niveau de risque de cancer colorectal, le type d'antécédents à risque de cancer colorectal et le statut professionnel des patients étaient significativement liés à l'adéquation aux recommandations. Conclusion : il semble exister une inadéquation du dépistage du cancer colorectal aux recommandations de la HAS chez une part non négligeable de participants. Cette proportion semble correspondre à celle observée dans les rares études internationales s'étant intéressées à la pertinence des examens de coloscopie. Elle pourrait être responsable d'une prise en charge sous-optimale des patients à risque élevé et majorer la probabilité d'effets secondaires inutiles chez les patients à plus faible risque. Son évaluation doit donc se poursuivre à plus grande échelle afin d'en déterminer plus finement l'importance et les déterminants.

It has been recognized for almost 200 years that certain families seem to inherit cancer. It is only in the past decade, however, that molecular genetics and epidemiology have combined to define the role of inheritance in cancer more clearly, and to identify some of the genes involved.
