Les chimiothérapies anticancéreuses constituent un des piliers dans le traitement des cancers. Elles présentent une toxicité importante connue depuis longtemps, mais ce n’est que récemment que les autorités de santé ont pris conscience de la nécessité de manipuler ces produits à risques avec beaucoup de précautions. Aujourd’hui, la préparation des médicaments cytotoxiques injectables est, en tant que préparation magistrale, réglementairement placée sous la responsabilité du pharmacien hospitalier. Du fait de leur caractère stérile et de leur dangerosité à la fois pour le personnel et l’environnement, ces préparations doivent être effectuées au sein d’une unité centralisée dans des locaux dédiés et avec un équipement approprié. La mise en conformité de l’Unité de Préparation des Chimiothérapies Anticancéreuses du CH de Flers avec la réglementation en vigueur, nous a conduits à l’acquisition d’un nouvel isolateur installé dans une salle à atmosphère contrôlée. En parallèle, l’informatisation du circuit des chimiothérapies, la définition des flux, la mise en place d’une nouvelle organisation ainsi que la mise à jour du système d’assurance qualité ont permis de sécuriser le circuit. Polyvalence, pluridisciplinarité et organisation sont les clefs indispensables à la réussite d’un tel projet.

La manipulation des cytotoxiques en milieu hospitalier pose les problèmes d'innocuité pour le personnel soignant et de la qualité de la préparation réalisée et administrée au patient. La prise en compte de la réglementation en vigueur a conduit l'hôpital Antoine Béclère a intégré dans son plan directeur la mise en place d'une Unité de Reconstitution Centralisée des Cytotoxiques (URCC). Le travail présenté dans cette thèse s'articule autours de trois grands axes. Le premier est un rappel de la réglementation de la préparation des produits toxiques. Le second est une étude bibliographique visant à définir les conditions de reconstitution, de dilution et de conservation des anticancéreux. Le troisième, est une analyse de la situation dans notre établissement avec une étude prospective sur le mode de préparation des anticancéreux dans les services de soins. Les résultats obtenus lors de l'étude réalisée dans les services montrent la nécessité de centraliser la préparation des anticancéreux. L'étude bibliographique réalisée nous a incité à opter pour une URCC équipée d'un isolateur en surpression placé dans une zone à empoussièrement contrôlé. En conclusion, cette thèse montre l'expérience de la création de l'URCC de l'Hôpital Antoine Béclère et l'intérêt de cette prise en charge pharmaceutique de la préparation des anticancéreux. La centralisation de la reconstitution des chimiothérapies et la mise en place d'un système qualité permettront d'assurer une sécurité de manipulation et une amélioration des soins dispensés aux patients.

La prise en charge médicamenteuse du patient est un processus pluridisciplinaire complexe comprenant de nombreuses étapes (prescription, préparation, administration des médicaments). Chacune de ces étapes comporte des risques pouvant engendrer des erreurs. Il convient de maîtriser l'ensemble de ce processus et pour cela de l'intégrer dans une démarche d'amélioration continue de la qualité et de gestion des risques. Au Centre Jean Perrin de Clermont-Ferrand, une nouvelle unité de préparation des anticancéreux injectables a été mise en place afin de répondre aux exigences réglementaires. Dans le cadre de la démarche d'amélioration continue de la qualité, une cartographie des risques a été réalisée. La mise en place de cette unité a entrainé une modification des pratiques et la mise en oeuvre d'une organisation différente. Elle a permis d'améliorer considérablement les points considérés comme critiques dans l'ancienne unité mais a aussi engendré de nouveaux risques qu'il a fallu maitriser. En effet, la mise à disposition d'une préparation de qualité, dans un délai acceptable, tout en respectant les Bonnes Pratiques constitue un véritable défi d'organisation pour les unités de préparation. Cette analyse des risques a présenté un intérêt majeur pour l'optimisation et la sécurisation de l'activité de l'unité et s'est d'ailleurs inscrite dans la démarche de certification de la nouvelle unité de préparation des anticancéreux injectables.

Evaluates the carcinogenic risks to humans posed by the use of four antiretroviral agents four DNA topoisomerase II inhibitors used in the treatment of cancer and an additional three pharmaceutical agents (hydroxyures phenolphthalein and vitamin K substances). The volume marks the first IARC evaluation of nucleoside analogs that act as antiviral agents. The evaluation responds in part to recent findings that zidovudine (AZT) an effective antiretroviral agent now being given to pregnant HIV-infected women to prevent maternal-to-fetal transmission of the virus is a transplacental carcinogen in mice. The opening monograph evaluates the carcinogenicity to humans of the antiretroviral nucleoside analogs zidovudine (AZT) zalcitabine (ddC) and didanosine (ddI) and the antiherpesvirus drug aciclovir. Of these aciclovir and didanosine could not be classified on the basis of available data. For zidovudine transplacental administration to mice resulted in an increased incidence and multiplicity of lung and liver tumours and in an increased incidence of female reproductive tract tumours in one study but not in another involving treatment at a lower dose. Despite observation of toxic effects in some studies of humans human carcinogenicity data were judged to provide inadequate evidence of carcinogenicity in humans. Zidovudine was classified as possibly carcinogenic to humans. Similar weaknesses in human carcinogenicity data for zalcitabine which consistently induces thymic lymphomas in mice resulted in its classification as possibly carcinogenic to humans. The second monograph evaluates four DNA topoisomerase II inhibitors: etoposide teniposide mitoxantrone and amsacrine. Of these etoposide - one of the most widely used and effective cytotoxic drugs in combination therapy - was classified as probably carcinogenic to humans and etoposide in combination with cisplatin and bleomycin was judged to be carcinogenic to humans. Teniposide was classified as probably carcinogenic to humans and mitoxantrone and amsacrine were classified as possibly carcinogenic to humans. Of the three pharmaceutical agents evaluated in the final monograph hydroxyurea which is widely used in cancer treatment and increasingly in combination with didanosine in HIV infection could not be classified. Phenolphthalein a widely used laxative now being withdrawn from the market in many countries because of toxicological concerns was classified as possibly carcinogenic. Vitamin K substances could not be classified on the basis of available evidence.
