The reorganization of the Department and establishment of a viable medical logistics system in competition with other commodities was a formidable task. The Army of the Potomac began with little to indicate its Medical Department would be equal to the calling, yet the subsequent assignment of Surgeon Johnathan Letterman as its Medical Director became the first step to excellence. His vision and organizational abilities set the standard for field medical support which would be institutionalized by the entire Union Army, ultimately providing exceptional support in the Civil War. Letterman's system was adopted by European armies and would become the basis for current medical logistics doctrine in Army divisions.

At the First Battle of Manassas, 21 July 1861, it became cruelly apparent that the Army of the Potomac did not possess the field medical support assets and doctrine to deal with the wounded in action (WIA) resulting from such large and destructive armies. Thus, under the direction of Surgeon Jonathan Letterman, Medical Director of the Army of the Potomac, dedicated medical evacuation vehicles and trained litter bearers were established along with very strict rules of operation. At the same time, Dr. Letterman established a more efficient and better quality field hospital. Even with a vastly improved field medical support system established in the Army of the Potomac, the medical evacuation above division level and field hospital support in total were inadequate during and after the Gettysburg Campaign. This was due to several significant factors, but three were of primary importance: first, General Meade's decision to allow dr. Letterman to bring up only a portion of the Army of the Potomac's field medical support system; secondly, the lack of organized medical services between the division and base or general hospitals, which left the care of the wounded at Gettysburg in disarray when Dr. Letterman moved most of the field medical support south with General Meade immediately after the Gettysburg Campaign; and the large number of casualties.

You will at once request the commander of your corps to direct the commissaries in his command to make the issues above mentioned, and it is enjoined upon you not to relax your efforts in this matter, so vitally important to the health of the troops, and the interests of the service. You will please direct each regimental Surgeon to state, in the "Remarks" on his weekly report of sick, how often these issues have been made during the preceding week, which you will condense in the " Remarks " on your weekly report to this office. You will please report to this office what action you have taken in pursuance of these instructions, and at the expiration of two weeks make a full report on the subject. "Very respectfully, "Your obedient servant, "JONA. LETTERMAN, Medical Director." On the 7th of February the Commanding General, at the suggestion of Colonel Clarke, ordered that subordinate commissaries should show good reasons why they had not issued fresh and dried vegetables, and fresh bread, whenever these articles had not been furnished by them to the troops.
