PURPOSE: To critically examine the results of a decentralized approach to Medicaid reform through managed care, in which program planning was conducted at the county level. METHODS: Investigators conducted a prospective embedded case study of the six rural counties of the western New York region. Qualitative primary, archival, and secondary data were collected from county managed care planners, state health and welfare administrators, and HMO representatives throughout a two-year study period in the form of interviews, focus groups, and reports. Invesigators synthesized the data into structured summaries for each county. County nmanaged care planners reviewed and verified the summaries. RESULTS: Counties developed Medicaid managed care plans that reflected local needs, characteristics, and interests. CONCLUSION: The flexibility of the decentralized approach permitted adaptations aimed at overcoming rural barriers and protecting rural health care delivery systems. The planning process, however, was time consuming.
